Job Description

POSITION SUMMARY: The Site Supervisor is responsible for the overall management of identified community residential service location(s), including the orientation and supervision of all assigned staff, medications oversight, medical appointments, food shopping/petty cash/menu implementation, fire drills, room care, outcomes implementation and documentation, physical site maintenance and vehicle issues.

DUTIES AND RESPONSIBILITIES:



  • Interview and hire to build and maintain a team of staff focused on Individual Support Plan (ISP) outcomes, program structure and client rights through ongoing support, individual supervision, training, and regular staff meetings
  • Provide timely feedback on Life Skill Associates (LSAs) and Lead LSAs job performance, including providing counseling and disciplinary actions as appropriate
  • Ensure assigned personnel maintain compliance with training and credentialing requirements including but not limited to completing site orientation before working at the home, completing 24 hours of training annually, and completing required annual training topics by coordinating employee worksite schedule to support participation in training activities
  • Provide ongoing guidance to staff regarding the daily care, instruction, and safety of persons served at assigned program site as outlined in the ISP
  • Ensure sites operate in accordance with the organization's quality standards for safety, cleanliness, and comfort and state licensing and accreditation standards by reviewing (including but not limited to) routine maintenance and cleanliness of vehicles assigned, environmental audits of physical site, staff documentation of daily shift activities and by delegating or performing corrective action plan completion and following up with site and maintenance staff accordingly
  • Communicate operational needs and status to Program Manager to maximize overall efficient and effective operations of the site
  • Direct and oversee LSA staff in the following to ensure:

    • Accurate and safe completion of medication administration and documentation including but not limited to medical appointment follow up, processing of medication scripts, picking up medication from pharmacy, updating Medical Administration Records (MAR), accurate completion of required paperwork by staff for submission to program management.
    • Effective utilization of petty cash to purchase items as directed by the Program Manager
    • Compliance with purchasing food for balanced nutrition and following the established menus
    • Laundering and upkeep of individuals' clothing and inventory of personal property including adaptive equipment owned by the individual
    • Proper utilization of Elwyn property to maximize use of resources for the individuals
    • Completion of monthly and new admission fire drills including documentation of the drills
    • Utilization and maintenance of adaptive equipment used by individuals supported
    • Planning and execution of community outings with the lead of the Program Manager
    • Implementation of care plans toward individual outcomes and support for specific basic individual needs
    • Completion of room care/chores and other aspects of the individual's health and safety requirements
    • Scheduling of, follow through for, and transportation to, medical appointments


  • Comply with budgetary constraints, including but not limited to management and reconciliation of petty cash, scheduling staffing hours limiting overtime whenever possible and maximize resources to fulfil community outing requirements, as identified by the Program Manager
  • Secure all in-house funds and budget funds in accordance with established program guidelines
  • Complete timekeeping activities for all assigned staff in accordance with established program protocols
  • Act as liaison to the community and the families of individuals in the absence of the program manager
  • Participate in the development and implementation of the ISP
  • Complete paperwork and maintain all documentation as required (and as applicable to specific program) and as determined by program management for the program site; e.g., daily logs, monthly progress reports, daily goal reports, assessments, daily financial transactions, MAR, behavioral data sheets, Incident Reports, Parent/Guardian/Other contact form, etc.
  • Aid in the resolution of problematic situations that may occur for persons served within a specific program or job site and notify program management of any unusual or emergency situations
  • Maintain adequate medications inventory levels to ensure immediate availability of medications.
  • Participate in the Interdisciplinary Team (IDT) process in conjunction with the program manager including but not limited to collecting data in preparation for the meeting, assisting to host the meeting, reporting issues in the event that the implementation of the plan is problematic
  • Complete MAR reviews weekly generating report for Program Manager
  • Serve as first point of contact during assigned shift/schedule for daily and emergency situations; i.e., carries phone during all work times to ensure immediate communication with others
  • Ensure proper staffing levels and work distribution within assigned program site and maintains levels of supervision as defined in Individual Service Plan (ISP)
  • Document and report performance-related issues as needed providing regularly scheduled supervision and performance improvement activities
  • Complete required trainings to become a medication practicum observer and maintain certification
  • Complete Page 2 of the incident report (follow up) for medication errors
  • Participate in nursing meetings
  • Ensure medical appointments are kept, maintain adequate staffing level to keep appointments, submitting/processing paperwork
  • Ensure census is set in electronic record and provide backup to lead LSA for census setting as needed
  • Remain current with own required training certifications, meets state-regulated licensing and regulatory accreditations, and adheres to mandatory requirements
  • Promptly reports any infraction of Elwyn policies to Program management
  • Provide on-call coverage as needed
  • Perform cardio pulmonary resuscitation (CPR), medication administration and crisis intervention techniques in accordance to agency trained protocol
  • Performs other related duties as assigned

DIRECT REPORTS: Life Skills Associates (LSAs) and Lead LSAs

CONTACTS: individuals supported, family members/guardians, community members/neighbors, monitors, medical professionals, and Elwyn employees

EDUCATION/SKILLS/EXPERIENCE REQUIREMENTS:



  • High School Diploma required, Bachelor's degree in business or human service field preferred
  • Two (2) years of previous experience in residential programs for people with disabilities
  • One (1) year supervisory and/or management experience
  • Experience with Microsoft Outlook, Word and Excel helpful
  • Ability to be flexible and adaptable in work schedules and work assignments as defined by program needs
  • Ability to effectively communicate both in writing and verbally
  • Ability to request, accept and utilize feedback and training appropriately
  • Ability to provide effective feedback, train, and delegate job assignments
  • Ability to assess and appropriately and timely address job performance
  • Ability to independently problem solve
  • Ability to perform cardio pulmonary resuscitation (CPR), and crisis intervention techniques in accordance to agency trained protocol
  • Experience using electronic client record a plus
  • Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
