Multidisciplinary care team including a dedicated Outcomes Manager. The leader will work with a team of Physical Medicine and Rehab Physiatrists. Care Conferences daily on unit with a multidiscipline approach. The nursing staff compliment is 5-6:1 ratio with 2 PCT's and a Unit Secretary and a full complement of PT, OT and Speech. The unit is connected to an acute care hospital that offers consultative services and dialysis to patients on rehab unit. The position reports to the AVP of Patient Care Services and up through the Chief Nursing Officer. Summary: |
