COMPENSATION AND BENEFITS

Our compensation package offers competitive hourly rates, time-and-a-half for holidays, and a generous paid-time-off program. POSITION SUMMARY

Provides low intensity progressive exercise and activity programs to older adult patients as directed by a Physical Therapist and/or responsible Nurse. Care includes intervention to maximize exercise and activity capacity and to promote and maintain patients' optimal activity, mobility, and independence. These services are provided as an adjunct to skilled rehabilitation and designed to decrease the detrimental effects of inactivity. The mobility specialist reports to the director of rehab, physical therapist and overseeing nursing staff.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Clinical and Communication:





  • Provides low intensity progressive exercise and activity programs to older adult patients in a safe and effective manner.



  • Employs strategies to gain patient's participation and engagement with activity.



  • Follows treatment plans as directed and in accordance with department procedures and standards.



  • Monitors patient's response to intervention and able to adapt strategies to meet the individual needs of the patient.



  • Maintains good judgment in difficult situations, asks appropriate questions when unsure of assignments, and takes initiative in areas expected of them. Able to work independently without direct supervision.



  • Schedules time to maximize patient participation in conjunction with other team members. Records specific activity and treatment times per established procedures.



  • Recognizes opportunities to assist the rehabilitation team and takes initiative to provide support.



  • Seeks and probes for information from nursing staff relative to the patient's mobility and self-care performance and in accordance with MDS procedures. Conveys gathered information to appropriate nursing and therapy staff.



  • Assists in the training of certified nursing assistant staff on the delivery of nursing restorative plans. Enlists support of other nursing leaders or rehabilitation professionals when needed.



  • Assists patients with moving in/out of bed, transferring from one surface to another, and walking safely.



  • Conveys feedback and information from patient and/or family to therapy and nursing personnel as needed.



  • Maintains positive relationships and rapport with co-workers, patients, family members, and building personnel, modeling respectful behavior by exhibiting professionalism with coworkers.



  • Maintains knowledge of and adheres to all applicable and established confidentiality standards including all HIPAA rules and regulations.



  • Demonstrates and understands the importance of and respect for the rights, dignity and individuality of each patient visitor and staff in all interactions.



  • Reports, as appropriate, actual or suspicious activity related to fraud and/or operational abuse of laws, regulations and standards.





Processes:





  • Limited documentation of patient treatments/interactions.



  • Reports to work on time and maintains regular attendance.



  • Maintains safe and clean work area and adheres to company safety standards. Complies with all Infection Control, Universal Precautions, and OSHA standards for the healthcare professional. Attends annual safety and infection control education required by agency.



  • Uses safety precautions as appropriate for patients, employees, public and self, e.g. proper positioning of self, good body mechanics, universal precautions,



  • Knows location of all emergency equipment in the facility, and is aware of established emergency and safety procedures. Develops and maintains own and patient/caregiver knowledge of emergency procedures.



  • Demonstrates good judgment when the environment is not physically safe for the staff or the patient.





Other responsibilities:





  • Attends in-service meetings as required.



  • Assumes responsibility for ongoing continuing education and professional development.



  • Assists with and participates in quality improvement program as required.



  • Performs other duties as assigned.

Qualifications

Minimum qualifications:





  • High school diploma and post-secondary course work in biological sciences, medical terminology, communication and conflict resolution, basic life support, first aid, or similar required.



  • Current CPR certification required.



  • 0-2 years related experience with exercise, activity, and/or older adults preferred.



  • Must be able to work independently, take initiative, and be dependable.



  • A passion for the value of exercise and activity in improving health, function, and quality of life is needed to be successful.





Preferred qualifications:





  • Certification as a certified nursing assistant, personal trainer, exercise coach, or similar recommended.



  • Associate/bachelor's degree in Exercise Science, other biological science, or psychology in lieu of other recommended certifications.
