As an integral member of the healthcare team, the Medication History Technician historyis responsible for collecting an accurate medication and allergy history for admitted patients in the Emergency Department and/or inpatient units. Information will be obtained by interviewing the patient, family members, retail pharmacy, nursing home documentation/staff, and any other available resources. The accurate home list will be documented in eCare to be used by physicians and other healthcare providers to determine treatment plans and medication orders during admission and post-discharge. Performs tasks that require higher levels of competency, including responsibilities of trainer and mentor, assists pharmacists with quality audit, develops and presents education to MHT/nursing relevant to medication history, etc. The technician will communicate pertinent information (i.e. medications that need to be clarified with the physician and/or other healthcare providers) to the supervising pharmacist.

Required: 1 year of experience working as a Medication History Technician, performing tasks that require higher levels of competency, including the responsibilities of trainer and mentor, assisting pharmacists with quality audits, developing and presenting education to MHT/nursing relevant to medication history, etc.

Preferred: PTCB Medication History Specialized Certificate

Schedule: Dayshift: Monday - Friday, 8:00am - 4:30pm. Rotating Major Holidays.
