As a Case Manager you will provide resources, education and support to clients. As well as develop and implement individualized care plans.

Primary Responsibilities:



  • Provide education and support families
  • Coordinate services within and outside of the clinic
  • Help clients access and utilize community resources
  • Act as a liaison between the client and service providers

Required Qualifications:



  • High School diploma
  • Experience with case management or other mental health related experience
  • Proven solid verbal and written communication skills


Preferred Qualifications:



  • Experience with couples and family counseling
  • Experience with children or adolescents
  • Experience with health psychology
  • Competent with a wide range of clinical requisitions
