Description

This position is responsible for providing direct patient care to patients at the SIU Center for Healthy Aging and Wellness and for assisting in teaching activities as they relate to learner programs under the Center. Duties will include a combination of patient care and teaching activities.


Examples of Duties

I. TEACHING (10%)
A. Teach and evaluate medical students, residents, fellows, physician assistants, and nurse practitioner students as they rotate through the educational program.
B. Role model provision of care by an advanced practice professional focusing on health education, health promotion, primary and secondary disease prevention.

II. SERVICE (90%)
A. Provide primary care services to patients at all Addiction Medicine clinic sites and serve as primary care provider (PCP) to patients who do not have a PCP.
B. Serve as part of the Street Medicine healthcare team to provide needed primary care services to the homeless and other vulnerable populations.
C. Provide prevention and health maintenance care to patients and families.
D. Development of a collaborative clinical practice model with faculty and community medical specialists focusing on division of responsibilities and appropriate referrals.
E. Provide continuity of care across the health care continuum for collaborative practice.
F. Participate in Peer review and Quality Improvement Projects for the Center.
G. Provide emergency services to patients as needed including those who are identified as having no primary care physician.
H. Attend faculty meetings and resident conferences.
I. Serve on the Center-wide Committees, as assigned.
J. Serve on school and department committees, as assigned.
K. Participate in community outreach projects.
L. Maintain competence in clinical practice using continuing education opportunities, professional conferences, and maintains licensure/certification.

Qualifications

A. Sensitive to the needs of underrepresented minority populations.
B. Graduate of an accredited Nurse Practitioner or Physician Assistant program at time of Hire. C. Received passing score on national certifying exam at time of hire.
D. Licensed or ability to be licensed in the State of IL as an Advanced Practice Nurse or Physician Assistant.
E. Licensed or ability to obtain a Controlled Substance License through Federal Drug Enforcement Administration
