*Graduate of an accredited school of nursing required.
*Current active licensure as a Registered Nurse in NH required.
*BLS required prior to hire.
*Minimum of 5 years of RN experience.
*2 years of Orthopedic nursing experience, with specific training for navigation or coordination of care for the Orthopedic patient preferred.

Licenses and Credentials

Registered Nurse [RN - State License] preferred

Basic Life Support [BLS] Certificate preferred

Additional Responsibilities:

  • Functioning on the multidisciplinary team as an advocate, interpreter, educator, and counselor for the orthopedic post-operative patient.
  • Responsible for ensuring all post-operative orthopedic patients receive quality and comprehensive service.
  • Providing expert nursing care which will include direct clinical practice, consultation, education, and research.
  • Coordinating patient care throughout the continuum in collaboration with the multidisciplinary team acting as a single point of contact for referring physicians, patients and caregivers to provide resources and assistance with clinical and supportive services.
  • Serving as a clinical resource with expertise in the defined specialty.
  • Serving as the key director of a specialty specific multidisciplinary program, developing and coordinating the model of care and services for the patient population.
  • Conducting the initial needs assessment and analysis to guide the establishment of a specialty specific multidisciplinary program with a design, structure and strategic plan to support and sustain services and care offered.
  • Collecting and synthesizing metrics to evaluate and direct the program goals including quality, service, patient satisfaction, appropriateness of care and cost effectiveness.
