At Healthcare in Action, we serve patients experiencing homelessness through an innovative "street medicine" approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems.

Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks.

The Manager, Care Management will enhance patients experiencing homelessness's ability to manage their health and live independently by overseeing and supporting the non-clinical team that conducts care management activities such as: enrollment, individual care planning, housing navigation, and other documentation. Co-owns program implementation success and ensures compliance with health plan guidelines for CalAIM programs.

This position would be responsible for the day-to-day operations as well as relationship management between the Health Plan CalOptima and Healthcare in Action (HIA) Street Medicine Provider. This role would be reporting to the HIA Regional Director and would be involved in spearheading successful implementation and achievement of performance measures, meeting program needs and developing efficient data collection and publication processes. Additionally, this role would be important as a cultural team leader and instilling our shared vision for street medicine both in our internal team as well as in partnership with other service providers.

Responsibilities include:



  • Manage role development, hiring, training, performance management, and coaching of Care Management team for assigned geography.
  • Ensure team is activating eligible patients/clients into CalAIM programs; drive team enrollment to meet/exceed targets.
  • Monitor direct reports individual caseloads to ensure they are meeting targets and that the overall team is reaching desired impact.
  • Monitor direct reports documentation and corresponding quality including referrals, assessments, individual care planning, and overall care coordination and housing navigation across multiple systems/platforms (e.g. verify monthly touch points, audit quality of notes, etc)
  • Co-develop and approve the patient's individualized care plan with direct reports utilizing evidence-based guidelines and clinical knowledge for clients by evaluating and conducting onsite visits, gathering assessment information, and identifying problems, goals and interventions.
  • Ensure direct reports deliver ongoing care coordination/care management by coaching, educating, navigating and referring to appropriate training and resources.
  • Coach and support Care Management team to assist patients in taking an active role in managing health and social needs by coaching, using motivational interviewing, and educating about self-management tools and strategies.
  • Exhibit strong interpersonal, critical thinking and analytical skills through positive communication with Care Management team and with internal/external stakeholders.
  • Demonstrate excellent organizational, decision-making and multi-tasking skills as demonstrated by problem solving and successful outcomes; identify barriers to work processes and lead process improvement efforts.
  • Lead various team meetings including interdisciplinary case conferencing.

  • Support financial reconciliation efforts related to CalAIM programming to maximize reimbursement and ensure organization's financial sustainability.



  • Special projects and other duties as assigned.



  • Actively support the achievement of HIA's Vision and Goals.

Qualifications and experience:




  • Bachelor's Degree in Social Work, Gerontology or related field of study, preferred.



  • Master's Degree in Social Work, Gerontology or related field of study, preferred.



  • 3+ years people leadership with some direct supervision experience



  • 3-4 years of experience in homeless care management field with a Bachelor's Degree



  • 2+ years of experience in homeless care management field with a Master's Degree.


  • A comparable combination of education/experience and/or training will be considered equivalent to the education listed above.
  • CalAim experience strongly preferred

  • Experience with street outreach is a plus.



  • Bilingual in English/Spanish preferred.



  • Demonstrated knowledge of care management principles: assessment, health and functional problems solving, individual care planning



  • Knowledge and/or familiarity with the principles of project and program management



  • Demonstrated ability to build and lead effective teams



  • Ability to work with all levels of staff, within and external to the organization to achieve goals.



  • Strong interpersonal skills, including excellent written and verbal communication skills.



  • Strong organizational skills; Ability to appropriately maintain confidentiality.



  • Strong analytical and critical thinking skills, required


  • Clean driving record

  • Requires proof of Valid Driver's License



  • Must maintain State Liability Insurance



  • Must have access to a Vehicle
