Description

1. Conducts individual assessments to identify skills, abilities, interest and support needs.

2. Develops employment plan with the individual, funding agency and significant others.

3. Performs job development to locate appropriate jobs in the community. Conducts job analysis, as needed, to determine specific job requirements and general work characteristics.

4. Assists individual in choosing appropriate job based on job analysis data and information obtained from the individual assessment.

6. Provides advocacy and training for employer and co-workers to promote individual integration in the workplace.

7. Prepares vocational plans, progress reports and other reports required for comprehensive case record maintenance, according to the standards of Fairfax County Community Service Board and Linden Resources.

8. Performs case management services and assumes primary responsibility for individual's program, case coordination, conduct and follow-up activities. Responsible for the safety of all clients supervised.

9. Provides for coordination of services with the individual's treatment team, funding agency and other community agencies.

10. Performs follow-up activities and provides ongoing support to the individual and employer to promote job retention, as assigned. Provides crisis intervention and retraining as needed. Maintains records on intervention activities, hours and expenses.

11. Performs other duties as may be assigned by the Program Director.

Requirements:



  • Bachelor's degree from an accredited college or university required, specialization in vocational rehabilitation or related field.
  • Previous business to business experience preferred.
  • Access to a motor vehicle and a valid driver license for consumer related visits.
  • Flexible schedule sometimes required.
