Position Description:

The Advanced Practice Provider (APP) works as a member of a multidisciplinary health care team who will contribute to excellence in program administration, patient care, patient safety, research, quality improvement, and teaching.

Our Community Physicians practice is seeking a Nurse Practitioner or Physician Assistant to support the Community Cardiovascular services primarily in a blended setting at both our Brookfield Heart and Vascular Clinic and Froedtert Menomonee Falls Hospital.

Primary Function/Responsibility

  • Collaborates with interdisciplinary health care teams in diagnosis and treatment of conditions requiring Cardiovascular intervention in a both an outpatient and inpatient environment as a member of the Community Physicians group.
  • Order, perform, and interpret screening and diagnostic procedures and tests, and appropriately communicates and documents information.
  • Facilitates and communicates with patient, family and staff to promote continuity of care across the continuum of care.
  • Team-based model and work with all providers on a rotating schedule
  • Rotational tasks/responsibilities (weekly rotational basis rotating between 6 APPs)
    • Primarily seeing established patients in a clinic setting
    • Conducting pre/post-op tasks
    • Responsible for stress test supervision and working with RNs in the Cath lab
    • Inpatient rounding and consults

Schedule: Support coverage in clinic, hospital and 24/7 Cath Lab

  • Primarily Monday thru Friday standard business clinical hours.
  • Travel as needed (weekly rotational basis)
  • Cath Lab (24/7) Rotational On-call
  • Saturday/Sunday onsite and off-site phone call (approx. 8-9 weekends per year & off the following Monday and Tuesday)

Minimum Education

Advanced Practice Nurse: Master's Degree or higher in Nursing

Physician Assistant: Bachelor's Degree or higher in Physician Assistant Studies


Advanced Practice Nurse

Advanced Practice Nurse Practitioner

Wisconsin RN License

Wisconsin Advanced Practice Nurse Prescriber License

Primary Care, Adult Care, Acute Care or FNP Certification

AHA ACLS, AHA BLS, DEA

Physician Assistant

Wisconsin Physician Assistant License

Certification by NCCPA

AHA ACLS, AHA BLS, DEA

Preferred Experience

*No experience required - Cardiology rotational preferred

Prior APP, RN, Cardiology or Hospital experience preferred
