The Department of Family Medicine is searching for an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to providing comprehensive care to patients with obesity, focusing on evidence-based lifestyle modifications, medication management, and coordination of care within a multidisciplinary team.
The APP will provide direct patient care through telehealth and in-person visits which include assessments, diagnosis, treatment planning, and ongoing management of obesity and related conditions. Provide medication management by prescribing and monitoring medications for weight management, including GLP-1s and other relevant treatments, in accordance with established protocols and guidelines. The APP will assist the patient with lifestyle modification by developing and implementing personalized nutrition plans, exercise recommendations, and behavioral therapy strategies to support weight loss and healthy lifestyle changes. The APP will provide patient education regarding obesity, its associated health risks, and available treatment options, fostering patient self-management and adherence to treatment plans. The APP will provide care coordination through collaboration with physicians, dietitians, psychologists, and other healthcare professionals to ensure comprehensive and coordinated care for patients. The position requires telehealth expertise. This position will hold both in person and telemedicine clinics. Proficiency in utilizing telehealth platforms, conducting virtual consultations, and ensuring patient safety and privacy is required. The APP will apply evidence-based principles and guidelines for obesity treatment, staying updated on the latest research and clinical recommendations. Experience in obesity medicine, nutrition, lifestyle modification, and/or related fields is preferred. The chosen candidate will provide comprehensive and personalized care to patients with obesity, utilizing evidence-based strategies for weight management and lifestyle modification. This individual will collaborate with a multidisciplinary team of physicians, dietitians, and behavioral health specialists to deliver holistic care. In collaboration with patients, pharmacists, care coordinators, and while navigating an evolving payor landscape, the incumbent will develop and implement individualized treatment plans, including nutrition counseling, lifestyle prescription, and medication management. Additionally, the incumbent will conduct telehealth consultations and provide ongoing support to patients throughout their weight management journey. Educate patients and families about obesity, its associated health risks, and available treatment options. Clinical: MINIMUM REQUIREMENTS Education: Master's Degree as an Advance Practice Provider - Nurse Practitioner or as an Advance Practice Provider - Physician Assistant Experience: One to two years minimum experience as an Advanced Practice Provider Nurse Practitioner or Physician Assistant preferred. Licensure: License or license eligible as an Advance Practice Provider (Nurse Practitioner or Physician Assistant) as set forth by the state of Virginia. If Nurse Practitioner, license to Practice as a Registered Nurse in the Commonwealth of Virginia is required. Must demonstrate and maintain all credentialing and licensing requirements of the UVA Medical Center, the Virginia Board of Medicine or the Virginia Board of Nursing, and the appropriate certifying bodies. Must be enrolled with all payers through Delegated Credentialing and can participate in Medicaid and Medicare for billing. Current Basic Life Support (BLS) required. Current PALS/NRP, as required specific to position. Advanced Cardiac Life Support (ACLS) as required. PHYSICAL DEMANDS Job requires standing for prolonged periods, frequently traveling/bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
