Responsible for the provision of pharmaceutical care for the Pharmacy Department through involvement in direct patient care, operational, educational and administrative Activities. Assists Manager and Director with daily operations and assumes responsibility in Manager or Directors absence.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Bachelor of Science in Pharmacy OR Doctor of Pharmacy (Pharm.D.) from an ACPE accredited school of pharmacy.

2. Must possess current and unrestricted license to practice pharmacy in West Virginia (or appropriate state licensure where services will be provided) or be license eligible. Licensure must be completed within 90 days of hire. If licensure is not obtained within 90 days of start date, associate will be notified that they will be released from their employment.

EXPERIENCE:

1. Five (5) years' experience as pharmacist in a pharmacy setting.

PREFERRED QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Pharm.D. from an ACPE accredited School of Pharmacy.

2. Board Certification.

EXPERIENCE:

1. Three (3) years' experience in leadership or management role.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. 1. Provides leadership and care for patients in assigned areas of responsibility; guides providers and patients on the best practices and established guidelines for care and assures safe and effective use of drugs through clinical involvement.

2. Reviews prescriptions issued by Physician, or other authorized prescriber to assure accuracy and determine formulas and ingredients needed.

3. Directs pharmacy workers engaged in mixing, packaging, and labeling pharmaceuticals.

4. Answers questions and provides information to pharmacy customers on drug interactions, side effects, dosage, and storage of pharmaceuticals, if counseling offer is accepted

5. Detects, reports, and helps to minimize or manage adverse drug reactions and medication errors.

6. Oversees the training and education for pharmacy tech trainees.

7. Assumes responsibility for and makes decisions related to the provision of pharmaceutical services.

8. Actively engages patients and clients with respect to customer service programs offered by the pharmacy.

9. Actively engages patients and clients with respect to program integrations offered by the pharmacy.

10. Identifies problems related to aspects of drug use control and offers constructive ideas for resolution.

11. Ensures that policies and procedures related to drug distribution are followed.

12. Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs.

13. Participates in the orientation and education of new department employees.

14. Maintains professional competence by attending and presenting at conferences, continuing educational programs, reading current professional literature and publishing clinical or practice-related experience or research.

15. Participates in administrative, quality assurance, performance improvement and research activities of the department.

16. Supervises technicians in drug preparation and distribution duties.

17. Participates in medication use evaluations, quality assurance and performance improvement initiatives.

18. Ensures Pharmacy staff are meeting Accreditation Guidelines and reviews Policies for suggestions

19. Instructs pharmacy technicians, and other pharmacy personnel on matters pertaining to pharmacy state/federal law.

20. Participates in planning and implementing changes in the pharmacy and improves productivity through participation in staff meetings and projects.

21. Assist Manager or Director in leading committees, projects, and workgroups.

22. Collaborates with other Pharmacists, technicians and supervisors on clinical and operational activities.

23. Responsible for Quality Improvement Projects that directly impact their area.

24. Holds routine meetings for pharmacists and technicians to provide updates and for quality improvement.

25. Assumes leadership role in absence of Manager or Director.

26. Aids with the training of new hires; pharmacist and technicians.

27. Provides input to Manager/Director in updating or writing departmental job descriptions.

28. Ensures services are in compliance with professional standards, state and federal regulatory requirements.

29. Acts in a lead or supervisory role in overseeing the workflow of pharmacy.

30. Provides ongoing feedback to technicians regarding their performance.

31. Assumes Role of Pharmacist in Charge if appointed by Manager/Director.
