Your role with us
As a Pharmacist with Salem Health, you will supervise the pharmacy technicians, pharmacy students, pharmacy interns, pharmacist trainees, and pharmacy residents. You'll monitor and/or manage the medication therapy of specific hospitalized or ambulatory patients. While consulting with patients, you'll coordinate with medical providers and other health care team members regarding medication therapy. In addition, you'll provide information and oversee procurement, storage, compound and distribute/dispense pharmaceutical products.What you'll doEvaluate patient electronic medical records to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medications (including over-the-counter and home therapies).Participates in Unit Care Rounds and as required review Pharmacy Technician patient interview summaries to obtain information regarding medication use, medication allergies and sensitivities.Evaluate and verify provider medication orders for appropriateness of drug, dosage, potential interactions, route of administration, stability and make recommendations for therapy changes as appropriate.Monitor and/or manage the ongoing medication therapy of specific hospitalized or ambulatory patients as assigned to assure optimum therapeutic results in the most cost-effective manner.Communicate with prescribing physician to resolve problems and document therapeutic interventions while assuming responsibility for accurate and timely drug distribution; provides medical personnel with medication information, drug therapy options, and pharmacokinetic dosing advice.Order and adjusts dosages, order laboratory tests, and administers medication under protocol according to established policies and procedures.Supervises and directs activities of Certified Pharmacy Technicians, Pharmacy Interns and Pharmacy Students as well as Pharmacy Residents who assist the pharmacist in the practice of pharmacy.Verifies accuracy of medication preparation and dispensing by pharmacy personnel; reviews patient's medical record to verify proper delivery and correct administration of medication.Serves as liaison to one or more nursing units or departments which use/or stock medications; responsible for establishing inventory and par levels consistent with patient care needs and completion of monthly floor inspections.
Background/experienceDoctor of Pharmacy required.Oregon Licensed Pharmacist required.BLS required 60 days from date of hire.ACLS required 60 days from date of hire.PGY1 Pharmacy Residency experience preferred.
