Job Description
POSITION SUMMARY: The Family Advocate is a liaison between Elwyn, Department of Behavioral Health and Intellectual Disabilities Services (DBHIDS), and families. This person either has or had a child/children with behavioral health care needs/diagnoses living in their home. The Family Advocate is an added support to the families in crisis who may need extra support, understanding of community resources, or help with navigating the behavioral health care system.
DUTIES AND RESPONSIBILITIES:


  • Assist parents/caregivers and families who have children with behavioral health care diagnoses by using lived experience to help parents/caregivers with navigating the behavioral health system, understanding services, and connecting to community resources
  • As a parent educator/coach using their own lived experience, support new families entering the behavioral health system to ensure families are informed, have someone they can talk to, and do not feel alone
  • Develop and maintain a resource library of current, helpful supports for children and families specific to Philadelphia North and Northwest areas and share these resources with families and make introductions/linkages where appropriate
  • Contact families via telephone, answer questions, assist with inquiries, and document information regarding resource requests appropriately to ensure timely follow-up on those requests; make home and community visits when necessary, and document all contacts with families to ensure timely and accurate record keeping of encounters with families
  • Represent Elwyn and Children's Mobile Crisis Services at community events; attend monthly meetings, trainings, and other group meetings to ensure knowledge of resources and best practices are up-to-date
  • Promote parent participation throughout the Individualized Education Plan (IEP) process, explain the importance of parental involvement, carryover activities, inclusion, and least restrictive environment to ensure families understand their role
  • Visit local community centers, schools, and churches to educate the community about the services they provide; attend resources events to gather information helpful to the families served by Elwyn so appropriate connections for support are made
  • Assist in building and maintaining working relationships with families, providers, and community advocates through regular communication and outreach
  • Attend psychiatric and psychological evaluations, interagency team, and school meetings at the request of the family to provide support and ensure families' understanding of the process and information and to ask clarifying question on behalf of the family
  • Perform cardiopulmonary resuscitation (CPR), and crisis intervention techniques using agency-trained protocols
  • Perform other duties as assigned



EDUCATION/SKILLS/EXPERIENCE REQUIREMENTS:


  • High School diploma or equivalent
  • Direct experience with a child with behavioral health care needs living at home
  • Direct knowledge of Philadelphia community resources
  • Demonstrated ability to work effectively as part of a team
  • Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
  • Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
  • Demonstrated strong attention to detail
  • Demonstrated strong time management and organizational skills
  • Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision
  • Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
  • Must be able to perform CPR, and safety care using agency-trained protocols
  • Demonstrated basic experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint; Access, Publisher and report-writer experience preferred
  • Experience using Electronic Health Record (EHR) preferred
  • Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
  • Fluency in a second language a plus
