Senior Manager - Senior Home and Continuum Services - Hybrid

VCU Health System is seeking a full time Senior Manager to manage clinical operations within its network of connected clinical resources engaged serving seniors and other at-risk patients across the Richmond region.
In partnership with the VCU Division of Geriatric Medicine, the Senior Manager for Senior, Home & Continuum Services manages and coordinates all administrative, managerial, operations and clerical functions required for the operation of highly complex patient care services provided within the VCU Health at Home and Continuum-based programs. The role oversees multiple clinical operations in an integrated network of connected resources engaged across the care continuum, including but not limited to: inpatient geriatric consultation and co-management, home-based primary care, short term SNF rehab, long term nursing home care, post-acute transitional telemedicine clinic, and Preference Bridge telemedicine clinic (advance directives).
Position is a key support resource for optimal management of the growing Medicare population affiliated with VCU Medical Center / Health System which incorporates strategic innovation that currently includes population health and value-based care (CMS ACO REACH, CMS Age Friendly Hospital) endeavors as well as telemedicine (Transitional clinic, Preference Bridge clinic); further innovation and growth is likely.
The operational leader for clinical services, this position requires the manager to serve as a resource in the service line for medical directors, staff, health care providers, management, and other disciplines, collaborating with Director and AVP in strategizing, planning, organizing, and controlling a broad group of continuum-spanning services and functions.

*7+ years of relevant experience required.
*Master's degree in Business Administration, Finance, Health Care Administration, Nursing or related field preferred.
*Experience in population health and/or value-based care preferred.
*Experience managing services for complex adult and/or geriatric patient populations preferred. Licensure, Certification, or Registration Requirements for Hire: N/A

Experience REQUIRED:
Seven years of relevant experience, preferably in an ambulatory setting.
Two years supervisory experience.
Experience PREFERRED:
Knowledge of ambulatory care and a background in clinic procedures.
IDX and Cerner experience a plus.
Education/training REQUIRED:
Bachelor's degree in Business Administration, Finance, Healthcare Administration, Nursing or related field or commensurate education and experience in lieu of a degree.
Education/training PREFERRED:
Master's degree in Business Administration, Finance, Health Care Administration, Nursing or related field.
Nursing or other clinical training a plus.
RN License highly preferred.
Independent action(s) required:
Has accountability for the performance and results of assigned patient care area(s).
Develops business, operational, production and organizational priorities in conjunction with Clinical Leadership for assigned patient care area(s).
Supervisory responsibilities (if applicable):
Provides leadership to Ambulatory Operations Management team consisting of both business and clinical leads within assigned patient care area or areas.

**This position is hybrid, 3 days in the office and 2 days remote**
