* Base Pay Range: $21.27 - $33.54 per hour, based on experience

DETAILED JOB DESCRIPTION

* Collaborate with RN Nurse Navigator(s) and clinical team with coordination of care along the continuum for specified disease processes or populations. Assure care pathway/order sets are followed per disease specialty.

* Act as a resource for patient, families, nursing staff and other health care professionals in area of expertise.

* Provide education on disease process and individualized treatment plan to patients and caregivers.

* Perform patient tracking and registry functions to insure care meets the requirements of value based contracts.

* Maintain the standards of nursing care and departmental criteria to assure alignment with MUHC mission and Goals.

* Provide triage/symptom management and route patient to appropriate level of care.

* Perform patient care duties and procedures following department policy, established protocols and standards of care or as directed by the RN.

* Conduct pre-visit planning for Care Management patients in conjunction with processes developed with other care team members including staff responsible for patient visit scheduling.

* Assist Nurse Navigator and clinical team to identify appropriate patients with chronic conditions for care management.

* Contact patients to schedule planned care visit, via letter or direct telephone contact as appropriate.

* Support primary care and subspecialty co-management with timely communication, inquiry, referrals and follow-up.

* Utilize and document through the Electronic Medical Record (EMR) all interactions with patients, caregivers/families to support a care process that is patient centered and moves toward a goal oriented plan of care.

* Perform other duties as assigned. This may include working in other locations to assist with coverage.

* May complete unit/department specific duties as outlined in department documents.

REQUIRED QUALIFICATIONS

* Missouri Board of Nursing LPN or Nurse Licensure Compact multi-state LPN. When primary state of residency changes, compact state LPNs must apply under new state of residency within thirty (30) days.

* Three (3) years of clinical LPN experience.

PREFERRED QUALIFICATIONS

* Experience in caring for patients with chronic diseases.

* Experience with navigation of local medical and social support systems.

* Additional license/certification requirements as determined by the hiring department.

PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met with or without reasonable accommodation. The performance of these physical demands is an essential function of the job. This position is very active and requires ambulating, reaching, and moving objects weighing up to and over 20 lbs.

This role also routinely comes into contact with patients who may have contagious illnesses. This position is in a health care facility open 24 hours a day, 365 days a year. Rotating shifts as well as holiday and work during inclement weather will be required.
