lives of older adults through community, choice, and continuing care in the Home Health License Practical Nurse (LPN) role. Under the direction of a registered nurse, provide high-quality nursing care to home health and hospice patients within the comfort of their homes (on and off retirement community). The LPN will work closely with patients, families, and other healthcare professionals to develop and implement individualized care plans. This role requires excellent clinical skills, strong communication abilities, and a passion for delivering personalized care to patients in need. Find your sense of belonging at Wesley! You will provide continuing care through these responsibilities: Conduct follow-up assessments of patients' health status and ongoing evaluations to monitor changes in condition. Follow care plans created by the Registered Nurse or other members of the Interdisciplinary Team (IDT) in collaboration with patients, families, and healthcare team members. Administer medications, treatments, and procedures as prescribed by physicians. Monitor and record vital signs, symptoms, and other relevant patient information. Provide education to patients and families on disease management, disease progression, medication administration, and overall wellness. Coordinate with other healthcare professionals, such as physicians, therapists, and social workers, to ensure continuity of care. Perform wound care, catheter care, and other nursing procedures per the care plan as established by the Registered Nurse. Assist with activities of daily living, including bathing, dressing, grooming, and mobility. Maintain accurate and up-to-date documentation of patient care activities and progress notes. Participate in interdisciplinary team meetings to discuss patient progress, goals, and discharge planning. Adhere to all regulatory standards, policies, and procedures governing home health care services. Provide emotional support and encouragement to patients and their families throughout the care process. Collaborate with community resources and agencies to facilitate access to additional support services for patients as needed. Learn and apply Company, Federal, State and Local rules, regulations, and policies. Other duties and special projects as assigned. Our requirements and qualifications for success: Completion of a qualified Licensed Practical Nursing (LPN) program and successful passing of the NCLEX. Current Licensed Practical Nurse (LPN) license in Washington State or out-of-state MSL If MSL, follow the Washington Nurse Practice Act. If MSL, complete Suicide Prevention Training in advance of hire per state regulations. If MSL, complete Washington State Board of Nursing (WABON) MSL demographic survey.Obtain CPR and First Aid Certification within 30 days of hire. Ability to obtain a Washington State Food Worker Card within 14 days of hire. Effective and comprehendible English verbal and written communication skills to accomplish charting, record keeping, and to comprehend written orders and procedures. Displays a personable and comforting personality to care for the residents. Use logic and reasoning to identify alternative solutions, conclusions, or approaches to problems. Physical, environmental, and mental requirements: Ability to give direct patient care, including participating in lifting, transferring, positioning, and assistance of residents with walking or in wheelchairs. Ability to lift and move patients as needed. Comfortable working in various home environments, including those with potential hazards such as pets or smoking. Capable of standing, walking, bending, and kneeling for extended periods. Good manual dexterity and visual acuity for performing nursing procedures.
