Overview To support the Mission of Helio Health, Inc., the CTI Team Leader will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a "critical time" transitional period in their lives. CTI is designed specifically to enhance the continuity of care during transition from hospital to community care. CTI Team Leader Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Critical Time Intervention Team Leader will oversee the new project implementation, be the point-person for engaging and providing education about the team and services provided to the community at-large. The Team Leader will oversee the team of two case managers. The Team Leader will facilitate weekly team meetings, monitor implementation, and be responsible for data collection and performance outcome reporting. The Team Leader will work on a Full-Time basis and using a hybrid work model, working in the community and out of Helio Health's Broome County region. Schedule: Monday- Friday, 8:00am-4:30pm, with some evening on call responsibilities. Pay Rate: $68,000.00 - $73,000 per year. In addition to Helio Health's comprehensive benefits package, Program Directors are eligible for a Sign-On incentive up to $3,000-$8,000 depending upon licensure! Responsibilities
