Responsibilities

At PrairieCare we are committed to providing exceptional mental health care to individuals facing psychiatric challenges. Our Partial Hospitalization Program is designed to offer intensive support while allowing patients to maintain connections with their community. We believe in the power of collaboration and are looking for a compassionate Social Worker/Case Manager who shares our mission of patient-centered care.

What You'll Do:
As a Social Worker/Case Manager in our PHP, you will play a vital role in supporting individuals with psychiatric needs. Your responsibilities will include:

  • Facilitating Admissions: Guide patients through the admission process, ensuring they feel welcomed and supported from the start.
  • Psychoeducation Groups: Lead engaging psychoeducational groups that empower patients with knowledge about their diagnoses, symptom management, and recovery strategies.
  • Treatment Planning: Collaborate with the primary Treatment Team, including psychiatric medical providers and therapists, to develop and revise comprehensive treatment and discharge plans.
  • Care Coordination: Serve as a key point of contact for patients and their support systems, ensuring effective communication and involvement in treatment plan decisions.
  • Discharge Planning: Develop and implement complex discharge plans, preparing patients for a successful transition back to their communities.
  • Advocacy and Support: Provide culturally appropriate, age-appropriate care while advocating for the needs and rights of patients throughout their treatment journey.

Qualifications

Requirements for Social Worker:

  • Licensure in the State of Minnesota is required
  • Bachelor's Degree in Social Work is required.
  • Master's Degree in Social Work is preferred.

Requirements for Case Manager:

  • Bachelor's Degree is required.
  • Master's Degree in Counseling, Marriage and Family Therapy, Psychology, Human Services, or a related Social Service concentration is preferred.

Pay range the social worker role is $54,080 to $75,795 depending on education and experience.

Pay range for the Case Manager role is $25 to $29 and hour depending on education and experience.
