Position Details

• Lead consumer engagement in community settings through the Supported Employment model, driving job development, placement, and coaching.
• Conduct vocational assessments and provide tailored skill development to help individuals achieve employment and educational goals.
• Collaborate actively with consumers and community partners,participatingin Community Support Planning to align support strategies.
• Build andmaintainrelationships with employers, providing ongoing support and guidance to ensure sustained consumer success.

Our Assertive Community Treatment (ACT) model supports individuals with serious mental illnesses who may not have experienced success orhaven'tfound effective support within traditional outpatient programs.We offer tailored services emphasizing relationship-building and active involvement in enhancing functioning and symptom management.

Services involve weekly home visits, provided either in individuals' residences or within the community,utilizingpersonal vehicles for client transportation and community-based tasks.

The ideal candidate willpossessthe following qualifications:

• Bachelor's degree. Preferred focus on rehabilitation counseling.
• 1-2 years of experience supporting individuals managing mental illness for candidates without the preferred bachelor's degree in rehabilitation counseling will be considered.
• Proficiencyin recovery principles and working within a recovery-driven system.
• Valid Driver's License. Travel is necessary to offer support across Dauphin County.
