Position Details

• Coordinates resources and provides information tailored to client needs.
• Efficiently documents all resource coordination activities.
• Coordinates referrals for client-specific discharge planning, ensuring active family participation in creating a comprehensive discharge plan that reflects their choice of service providers and community resources.
• Monitors record-keeping, compliance, and remaining authorized units, ensuring client charts are current and include necessary documentation, such as incident reports within 24 hours when required.
• Assesses, monitors, and documents safety concerns.
• Completes case documentation in compliance with standards set by Merakey, county MHDS/CASSP, OMHSAS, and other relevant licensing, regulatory, and payor organizations.

Earn: $20.67+ per hr.
