Blended Case Manager

We are seeking aBlended Case Managerto join our team in ourWarminster, PAlocation.

The Blended Case Manager will:

• demonstrate a Rehabilitation/Recovery attitude toward service.
• provide consumer-driven strength-based approach to case management and crisis services.
• act as the primary case manager for assigned consumers.
• will conduct minimal bi-weekly support contacts at the client's home or wherever most appropriate.
• complete consumer-driven service and safety plans.
• coordinate benefits, housing, and all other aspects of service delivery.
• coordinate meetings with providers that also provide services for the consumer.
• ensure Progress Notes are completed within 72 hours of service delivery and that weekly productivity paperwork is submitted per timelines.
• ensures that paperwork for their consumers is current and not expired.
• make attempts to locate clientthrough family, friends, or relatives if unable to contact the client.

Earn $23.75 with the selection of our Enhanced Pay option.

Our ideal candidate will possess the following skills:

• 2 years human services experiencepreferred
• Valid PA Driver's license

• A high school diploma and 5 years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989;

or

• A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years' experience in direct contact with mental health consumers;

or

• A bachelor's degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling or education-Preferred
