EXPERIENCE AND EDUCATION Required Education Master's Degree Masters in Social Work. Graduate of an Institution certified by the Council of Social Work Education. Experience 2 years related experience. Licenses and Certifications (LMSW) LIC MASTER SOCIAL WORKER by the Texas State Board of Social Work Examiners. Temporary license is acceptable during first 90 days of employment. within 90 Days (BLS) BASIC LIFE SUPPORT BLS not required in transplant departments Upon Hire JOB DUTIES Participates in multidisciplinary rounds and proactively determines patient needs for discharge transitions including transportation, DME, LTAC, SNF, home health agencies and hospice. Utilizes crisis intervention and problem solving skills to facilitate and enhance patient and family's ability to meet acute and ongoing medical and psychosocial needs. Demonstrates ongoing familiarity with community resources to facilitate positive patient and family outcomes agencies to assure appropriate client access. Maintains effective documentation including tracking and referral information; consistently meets department and UTSW documentation standards. Identifies and reports situations or cases to appropriate governmental agencies as legally required and in compliance with organizational policies and procedures. Serves as an active participant of the health care team, communicating effectively with other health care providers, patients, families, other hospital personnel, and visitors. Identifies the need for professional growth and seeks appropriate development opportunities. Provides education to staff, patients and community resource affiliates. Maintains system for tracking placement of patients after discharge on monthly basis. Has knowledge of Department and Unit Performance Improvement program and is aware of expected participation.
