The Director of Population Health Management plays a strategic and leadership role in designing, implementing, and overseeing assigned programs and teams that optimize the health and well-being of targeted populations. This position requires a comprehensive understanding of healthcare delivery, standardized processes and protocols, innovative technology, data analytics, and a commitment to addressing social determinants of health to address healthcare challenges effectively.
- Leads and inspires a team, fostering a culture of collaboration and innovative initiatives that align with the organization's overall mission, vision, goals, and strategic plan.
- Works with departmental leadership and clinical teams to formulate growth strategies and identify business opportunities and operational goals.
- Directs growth initiatives and development of Population Health IHM/RPM and other programs as assigned by the Administrator.
- Ensures innovation, technology, data feed systems, and regulatory and RPM billing requirements are effectively managed.
- Facilitates and leads multi-disciplinary groups to develop and implement business and growth plans.
- Monitors the development of long-range plans and annual budgets for assigned programs or projects.
- Helps to develop partnerships with various entities, both internal and external to the department, which align with strategic planning priorities. This may include new business ventures, partner affiliations, provider outreach, grant opportunities, and network/market expansion.
- Assists in enhancing standard processes and integrating outpatient and inpatient services related to all Population Health program areas.
- Collaborates and supports goal setting and metrics that align with system strategy and monitors for performance.
- Works to retain and achieve organic growth within current referral patterns and identify areas to acquire new sources of patient volume, revenues, and program opportunities throughout UVA Health, UVA Community Health, UPG, and other UVA Health entities and external partnerships for patient care.
- Collaborates in developing and implementing strategies to reduce the cost of departmental services without compromising patient services.
- Coordinates and monitors the development of long-range plans and annual budgets for assigned programs or projects.
- Responsible for ongoing management of existing business and standardized, evidence-based workflows, swim lanes, processes, protocols, and practices for other interdependent operating units. This includes pharmacy, patient progression, and post-acute responsibilities as assigned.
- Manages Population Health Lead and Care Manager teams' recruitment, performance, and retention activities, as well as scheduling, approval of payroll, and emergent needs.
- Ensures training assignments for all new and current team members (lead and care managers) by the role, responsibilities, annual training, and other UVA Health requirements.
- Conduct QI audits in Epic and other platforms to ensure continuous improvement efforts and correct documentation of all lead and care managers.
- Manage crises and navigate change, ensuring programmatic efforts remain effective and responsive to evolving needs.
- Stay abreast of industry trends, regulatory changes, and best practices.
- Other duties as assigned.
Position Compensation Range: $93,142.40 - $149,032.00 Annual
MINIMUM REQUIREMENTS Education: Bachelor degree required; Master's degree must be obtained within 4 years of hire or transfer. Experience: Five years of progressive leadership demonstrating expertise in population health, program, and/or business development within the healthcare industry. License/Certification: None.
PHYSICAL DEMANDS This is primarily a sedentary job involving extensive use of desktop computers. The job does occasionally require traveling some distance to attend meetings, and programs.
