The Brook - Dupont, located in Louisville, KY, is an 88-bed hospital offering a full continuum of inpatient, extended care, partial hospitalization and outpatient services for children and adults.

The Clinical Therapist is a member of the multidisciplinary treatment team who will be a part of our Outpatient Team. Major job functions include: obtaining psycho-social assessments; conducting family/group therapy on assigned cases; complete documentation per hospital policy and procedure; case coordination; coordinate discharge planning; assist in developing and maintaining community resource information; performs marketing functions as assigned; assists in the maintenance of the program environment in compliance with policies of safety and security; ensures good clinical judgment to ensure optimum patient care; participate in continuing education activities.

Job Duties

  • To provide family and group therapy on assigned cases, including obtaining psychosocial assessment information, referral source contact, case coordination and discharge planning
  • Assured family therapy is conducted when indicated by treatment team
  • Follows up with patient and family if pertinent on all missed sessions
  • Ensure that the therapies conducted are consistent with the goals and philosophies of the program, and supportive of each patient's individual treatment plan
  • Documents appropriate assessments and notes in patient chart as per hospital policy and procedure
  • Charting is clear, concise and reflects weekly treatment plan goals/objectives movement on a timely basis
  • Serves as Case Manager for assigned cases
  • Takes responsibility for coordinating the treatment team process

Requirements

Education: Required: Master's Degree from an accredited school in Behavioral Sciences

Licensure/ Certification: Must be licensed or under supervision in state of Kentucky (CSW/LCSW, CADC/LCADC, MFTA, LMFT, LPCA, LPCC, LPA, LPP)

Experience:Preferred: One to three years' experience
