Department:REP | CS North East JonesboroDepartment's Website: Summary of Job Duties:The Nurse Educator will work within the identified Family Medicine Clinic to implement and promote an Evidence-Based program to maintain a Patient and Family Centered Care approach. This role involves developing a clinical strategy for Well Child Checks (0-5 years) pre-visit planning, coordinating the transition of care for pediatric patients, and overseeing Community Health Worker call encounters to direct patient follow-up care. The Nurse Educator will effectively collaborate with nursing staff at all levels and participate with the clinic's complex care team to ensure optimal care for high-priority beneficiaries. The Nurse Educator will also support the Family Medicine Residency by providing pediatric/prenatal knowledge and skills through daily clinical encounters and participating in residency and nursing learning activities.
