Overview

Peer Recovery Specialist

Full-Time; FTE 1.0

Monday-Friday; 8:00am-4:30pm

Moline, IL

The Peer Support Specialist is indirectly supervised by the Department Manager and provides care to consumers utilizing the Robert Young Center principles of Trauma Informed Care. The Peer Support Specialist serves as a member of the Assertive Community Treatment Team (ACT) and provides recovery oriented in-home and community-based services for adults with severe mental illnesses. The Peer Support Specialist provides comprehensive case management and psychosocial rehabilitation through an outreach-oriented set of service activities. The Peer Support Specialist provides crisis intervention, maintain accurate clinical documentation, and demonstrate Trinity's principles and values when interacting with clients, families, visitors, physicians, and coworkers.


Responsibilities
  • Collects information necessary to develop an accurate assessment of each consumer
  • Participates in the development of the consumer treatment plan.
  • Provides mentoring, individual and group support, and teaches self-advocacy and motivation through feedback.
  • Provides assertive community outreach and sustained efforts to engage individual clients.
  • Provides linkage to mental health services and programs including but not limited to psychiatric services, medication management, and representative payee.
  • Evaluate the consumer's treatment plan through follow-up assessment and recommends adjustments based on clinical findings.

Qualifications

Education:

  • Requires GED or High School Diploma.
  • A Certified Peer Support Specialist (PRS) In State of Iowa is preferred.

Experience:

  • Must able to read, write, and communicate effectively.
  • Must have a lived experience with mental illness.

License(s)/Certification(s):

  • Meet all requirements of the Appalachian Consulting Group Peer Support Training Model by no later than 6 months after the date of hire.
  • Crisis Prevention Institute (CPI) training within 6 months of hire.

The salary range listed does not include other forms of compensation which may include bonuses/incentive, differential pay, or other forms of compensation or benefits that may be applicable to this role.
