Provides pathway for patient care and throughput. Supervises Care Coordinators/Discharge Planners. Facilitates problem-solving, clarification of regulatory and Carilion policies and procedures, may manage small patient load. Works in interdisciplinary team to complete treatment plans, identify patient resources, work with community providers to assure continuity of care, participates in departmental huddles.
- Provides leadership to patient care team to achieve positive patient outcomes. Follows-up on matters to ensure they are successfully resolved. Attends departmental and organizational management meetings.
- Assists with and/or prepares work schedules for work team. Insures adequate coverage, equitable treatment of staff, and mentors work hours. Addresses patient and staff issues, contacting the manager/director as necessary and/or apprises them of situation.
- Provides assessments of patients and families to identify social, emotional and environmental strengths and problems related to their diagnosis, illness, treatment and life situation.
- Assists Manager/Director in assuring staff competence and contributes to staff performance development. Identifies areas of improvement and development activities. Provides input to managers/directors developmental plan for each team member. Works with team members on areas of development goals and follows up to ensure employee progress. Participates in employee performance evaluations by providing feedback to manager/director. Provides employee coaching and feedback on a regular basis.
- Works collaboratively with treatment/care teams, community professionals, multi-disciplinary teams.
- Provides/arranges staff and patient education for issues related to the adaptation of patient's illness or situation.
- Works with staff members to create an environment which is conducive to patient satisfaction and to enhance the quality of patient care. Conducts individual/team meetings with team, emphasizing team communications, listening to needs and issues, follows up on issues with team members and management. Promotes positive work environment, holds employees accountable for conduct and interpersonal skills, and encourages staff development of communications.
- Plan of care promotes compliance with all applicable regulations as well as consideration of appropriate resource utilization. Advocates on behalf of patient. Utilizes electronic programs to facilitate services, placements and documents in these programs to meet documentations standards.
- Assesses patient's need for non-medical services within the parameters of the treatment plan.
- Formulates and develops comprehensive treatment/discharge plans. Coordinates plans with the interdisciplinary team and makes recommendations for further treatment for the patient to achieve his/her optimal level of functioning. Evaluates the process at appropriate intervals and modifies as necessary. Monitors length of stay/service.
- Oversees the arrangements and coordination of discharge/treatment plans post hospitalization or service delivery.
What We Require:
Education:Bachelor's degree or four years of equivalent experience in human services or related field
Experience: Experience in care coordination and in a health care setting preferred.
