Summary:

This position is for a licensed practical or registered nurse responsible for clinical coordination of a designated patient population.

Responsibilities:


  • Demonstrates knowledge and skills necessary to provide services based on the physical, psychosocial, educational, safety, and other related criteria appropriate population in his/her assigned area
  • Maintains required core competencies.
  • Complies with set Policies and Procedures (i.e. name tag, dress code, parking, smoking, etc.)
  • Communicate additional staff or staffing needs to the Joint Center Director
  • Act as the unit resource for support services and departments (i.e.: Coordinator, admitting department, emergency department, etc.).
  • Act as the unit resource for the multidisciplinary team (i.e.: Physicians, Social Worker, Chaplain, etc.).
  • Assists in the development of policies and procedures for the department and the implementation of the new strategies and procedures.
  • Coordinate elective total joint arthroplasties with orthopedic surgeons, Internal Medicine, orthopedic clinics to include, but not limited to: preoperative joint class, preoperative clearances with reference labs/consulting physicians, surgery departments, anesthesia dept.
  • Coordinate and conduct weekly presurgical interdisciplinary meetings to include: clinical nurse navigator, assistant director of joint center, anesthesia, OR Director, case manager, physical therapy, occupational therapy and any ad hoc team members deemed necessary.
  • Work closely with the medical director of the program to ensure quality patient outcomes* Work closely with all departments necessary in the preoperative and postoperative care of the patient to ensure quality patient outcomes
  • Maintain quality data deemed necessary by the quality department for reporting purposes as requested by the Joint Center Director
  • Perform preadmission intakes on patients prior to joint class
  • Participate in quality improvement monitoring programs as requested
  • Work with the medical director/internal medicine specialist on day of preoperative clearance acting as nursing support staff

Requirements:

Education


  • High School diploma or equivalent.

Experience


  • Demonstrated leadership and managerial ability should be evident as well as, documented progressive acute care nursing experience.
  • Performs related responsibilities as assigned.
  • Candidate must be able to communicate effectively in English.

Licenses, Certifications, or Registrations


  • LicensedPractical/Registered nurse with current New Mexico license
  • Basic Life Support (BLS) certification and any other departmental certifications required by the hiring department.
