The community wellness advocate (CWA) II will develop and implement programs and activities based on population health needs in partnership with community partners and providers to promote healthier lifestyles and improve health outcomes. The CWA II will implement innovative strategies to engage members in the communities where they live, work and gather to connect them to care and appropriate CDPHP resources. This position will provide expertise in developing and implementing outreach strategies for targeted member engagement at community-based outreach locations, events and through community partnerships. The CWA II will also provide representation on assigned workgroups and coalitions, as well as regular data entry, reporting and evaluation of targeted member outreach efforts.

Qualifications:

  • Bachelor's degree or four (4) years of equivalent experience required.
  • Master's degree preferred.
  • Minimum of two (2) years' experience in a consultative position with specialization in health education, population health management programs, quality assurance monitoring, and/or training is required.
  • Prior experience working in a direct human services capacity is required.
  • Working knowledge of the social determinants of health required.
  • Demonstrated knowledge of HEDIS/QARR & NCQA preferred.
  • High level analytical skills required. Experience working with health care data preferred.
  • Computer experience in a windows-based environment and knowledge of Word, Excel and Access required.
  • Experience with Government or State Programs population preferred.
  • Experience in a health insurance setting preferred.
  • Excellent verbal and written communication skills required.
  • Experience utilizing data management systems including Excel and Access.
  • Demonstrated problem solving ability is required.
  • Must have access to reliable transportation. If driving vehicle for CDPHP business, employee must have valid driver's license, current inspection, and appropriate insurance coverage in effect at all times.
  • Must be willing to travel and conduct/observe educational visits to provider offices, employer groups, and community events on behalf of CDPHP. Must be available to travel and attend company related events during the workday, evenings, and weekends.

Annual salary range: $63,000.00 - $80,000.00

CDPHP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. In addition to cash compensation, CDPHP employees may be eligible for an incentive payment, a discretionary cash reward based on employee and company performance. Some roles may also be eligible for overtime pay.
