UNM Medical Group, Inc. is hiring for a Counselor Social Worker I to join our Behavioral Health Clinic. This opportunity is a part-time (0.5FTE 20 hours a week) day shift opening that is 100% on-site in Rio Rancho, New Mexico.

Minimum $56,173- Midpoint $70,217*

*Salary is determined based on years of total relevant experience.

*Salary is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.

Summary:

Under supervision of a licensed independent behavioral health provider, provides a range of services including psychosocial evaluation/assessment, treatment planning, therapeutic intervention, referral to other needed behavioral health and related services, coordination of care with other providers, and participation in a team care approach as necessary, in a high-intensity clinical environment which requires a high degree of professionalism, independent decision-making, and thorough, timely documentation of services provided.

Minimum Job Requirements of a Counselor Social Worker I:

Master's degree in Counseling, Social Work, or a related degree. No experience required. Licensed eligible Counselor or Social Worker in the State of New Mexico, or eligible for license in the State of New Mexico required. Completed degree from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. Verification of education and licensure will be required if selected for hire.

Duties and Responsibilities of a Counselor Social Worker I:

1. Facilitates client access to community resources, including locating recreational activities, housing, food, clothing, school programs, vocational opportunities or services, and relevant mental health services; assists client to develop natural resources and make contact with social support networks.

2. Provides information, education and coaching in life skills (e.g. wellness promotion, parenting skills, job-search strategies, social skill building, and brief interventions) for clients and their families in a compassionate, professional and culturally responsive manner. Will also provide health literacy training as well as disease management skills.

3. Engages clients in a process of ongoing information gathering and decision making in order to empower the client to identify their own goals, strengths and challenges.

4. Provides brief intervention to clients and families as appropriate to the position; contribute to that development of treatment plans, discharge plans, and documentation for coordination of care, follow-up care programs; provides therapeutic crisis intervention and emergency services as required.

5. Collects data about patients through interview, case history, evaluates data to identify causes of problems and to determine proper therapeutic care necessary to engage patient in primary care or referral to other specialists.

6. Consults with team members as appropriate, huddle and as needed to facilitate coordination of care; refers clients to appropriate service agencies as required.

7. Arranges for emergent, urgent or therapeutic interventions as appropriate for patients or clients in a crisis condition and for those with serious disturbance problems.

8. Establishes and maintains case files, assessments, brief intervention information, referrals, and other related documents for the treatment of clients.

9. Consults with other legal and treatment agencies and individuals in relation to patient/client records, rights, and responsibilities.

10. Performs miscellaneous job-related duties as assigned.

About the Behavioral Health Clinic - Rio Rancho:

The Behavioral Health Clinic has an interdisciplinary team providing accessible behavioral health care to the Sandoval County community, while also coordinating care with integrated behavioral providers at the neighboring Sandoval Regional Medical Center and nearby Grande Primary Care Clinic. Team members work collaboratively across behavioral health and related disciplines, including psychiatry, psychology, nursing, primary care, clinical social work, counseling, and peer support.
