What You Will Do - Essential Functions
The Community Health Worker (CHW) will work closely with patients in home-based medical practices to address both clinical and social health needs. Reporting to the Community Health Manager, the CHW builds trust with patients and their families, helping them navigate challenges such as transportation, food, and financial issues that impact their health. They will conduct in-person, telehealth, and telephonic visits, assess social support needs, and collaborate with care team members to support patient action plans and interventions. The role is field based with some administrative duties, requiring strong communication and community engagement skills.
- Patient Engagement & Relationship Building: Engage patients in various community settings (home, facility, clinic) to build trusting relationships and assess social and behavioral health needs through tools like PHQ-9 and GAD-7 questionnaires.
- Social Support & Referrals: Assist patients in addressing social needs (transportation, housing, food, financial assistance) and respond to referrals related to barriers to care, behavioral health issues, or changes in care needs.
- Health Education & Chronic Disease Management: Provide culturally relevant health education and support patients in managing chronic conditions such as depression, anxiety, heart failure, diabetes, and COPD, with a focus on lifestyle modifications.
- Collaboration & Documentation: Collaborate with the care team in case conferences and document follow-up actions in the electronic health record while maintaining an up-to-date community resource directory.
Who You Are
- Required Qualifications
- Longtime resident of their local community with great knowledge of local community resources
- Preferred 2 years or more of experience in health or social services
- High school or GED required
- Excellent oral and written communication skills & ability to carry out written and oral instructions
- Strong advocacy skills, including the ability to advocate for the patient
- Strong interpersonal and relationship building skills
- A knowledge of motivational interviewing preferred
- Ability to work independently as well as with a team with a strong desire to build collaborative relationships with all members of the field and regional teams
- Excellent time management and organizational skills are required
- Working knowledge of computers with an ability to navigate and use an electronic health record for effective and efficient documentation
- Ability to work weekends and evenings if required
- Bi-Lingual based on prevalence of local language
- Driver's license requirements: licensed for a minimum of 3 years without restrictions; no DUI or other felony driving conviction in the past 7 years.
- Travel required up to 60%
- Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.

- Ability to complete physical requirements of the job as necessary for in home visits including navigating stairs and a variety of dwelling conditions and sit and stand.
