Job Details Responsible for performing clinical assessments of patients to determine their candidacy for acute rehabilitation services and their appropriateness for admission.Job Description Essential Functions * Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson. * Facilitates the admissions process through the screening of patients for medical necessity and appropriate level of care. * Utilizes computer skills to view EMR. * Interfaces with insurance carrier to clear coverage details prior to patient admission if necessary. * Educates family preadmission while evaluating disposition plan for adequacy. * Initiates and builds relationships with referring physicians, hospital discharge planners and nurse clinicians to build referral base, as well as create seamless communication system to better serve patients referred. * Demonstrate expertise in analysis, decision making, time management, oral and written communication as well as computer literacy. * Performs other duties as assigned. Experience: Only years of experience in a particular setting, e.g. "5 years of experience in a doctor's office required." Education: Graduate of an accredited school of nursing with RN licensure. BS in nursing or another related field preferred. MSW with appropriate clinical experience will be considered. Experience: Minimum of 2 years clinical experience. Preferred experience in rehabilitation of chronically ill or catastrophically injured individuals.
