Responsible to the Program Director for oversight of services to homeless and those at imminent risk of homelessness in Rock and Walworth Counties. Individual will coordinate the delivery of effective case management services and navigation to ensure program integrity.
NRSA Funding: Responsible for providing assistance to residents living in the outlined target area defined in the City of Beloit NRSA plan. Conducts analysis of needs and provides resource referral services to all clients by facilitating and managing the coordination of agency and community resources available to clients. Promotes and maintains appropriate professional boundaries. * Problem Solving: Identifies problems, secures relevant information and implements solutions. * Planning and Organizing: Establishes and manages work priorities; efficiently allocates time and utilizes available resources appropriately; effectively handles multiple assignments. * Attention to Safety: Assists in the creation of a safety culture by working in a safe manner; reports unsafe situations and accidents; follows safety procedures; requests and uses safety equipment and safety techniques; and participates in safety training. * Attendance/Punctuality: Team member is on time and prepared for meeting with participants and stakeholders internal and external to the organization Case Management and Resource Navigation * Provide supportive services as needed according to funding source guidelines and budget. * Responsible for adhering to and reporting on program outcomes to the Program Director. * Aware of and strive to attain housing program performance measures per contract and/or initiated by the funder. * Responsible for complying with Agency and Department programs and policies. * Provide advocacy for participants as appropriate and make referrals to community services as needed. * Complete required forms, paperwork and documentation associated with operation of program services. * Complete data entry of participant information, reviews, and case notes into HMIS, CAP60, and/or other database in a timely manner. * Maintain confidential individualized files for each participant with all program-required forms/paperwork and up to date case notes as needed. * Meet face-to-face or by phone with participants at a frequency as required by program funder. * Assess participants' strengths, needs, and goals and develop correlating Housing Stability Plans (NRSA). * Support and monitor participants in achieving action steps toward self-sufficiency goals. * Assist participants in developing skills to coordinate own transportation. * Provide follow-up services for participants based on funder expectations. Coordinator Responsibilities: * Provide support and assistance to other housing staff as needed. * Provision of guidance on spending to meet budget and funder requirements. * Work with Program Director to identify areas of concern related to overall provision of effective case management services, services to specific participants and program service-related challenges or barriers. * Assist with the development and implementation of program improvements. * Provide support on funder applications, presentations and monitorings. * Assist in the issuance of timely and comprehensive reports as assigned. Internal/External Partnerships * Build and maintain working relationships with other service providers related to service delivery on behalf of Community Action programs. * Coordinate participant program services with other internal and external service providers. * Provide the school districts/early childhood education programs with contact information and connection to Coordinated Entry. * Support document collection for funding proposals, funding source reporting and provide public speaking requests as needed. * Develop and maintain positive working relationships with funding sources, community partners, participants, and peers. * Consistently review strategy and objectives for ongoing program development to develop systems to improve program operations, coordination, and collaboration among staff. * Attend staff, department, agency, and local community meetings as required. Education and Experience * A bachelor's degree in social work or a related field or * Four years case management experience working with low-income individuals/families and poverty issues, including housing instability, in a progressive leadership role. * Appropriate equivalent combination of education and work experience may also be accepted Language Skills * Requires excellent listening, verbal, and written communication skills, with strong attention to detail and accuracy. * Ability to establish rapport and relate sensitively to multiracial and multicultural individuals. * Ability to effectively present information in one-on-one and small group situations to participants. Mathematical Skills * Knowledge of basic math: add, subtract, multiply, divide, fractions, and percentages. Reasoning Ability * Ability to interpret a variety of instructions in written, oral or diagram form, to solve problems requiring decision-making skills within department guidelines. Certificates, Licenses, Registrations * Current social work certification from State of Wisconsin preferred. * Access to reliable, licensed, insured driver and transportation required. Other Skills and Abilities * Must possess strong interpersonal, organizational, and independent work skills. * Ability to work independently and collaboratively with diverse staff and participants and provide services in a structured, yet individualized, creative, and non-judgmental manner. * Must possess experience with office equipment and procedures and basic budget and financial transaction procedures. * Must have computer background and experience with Microsoft applications and internet. * Ability to proficiently utilize Management Information Systems, data entry. * Knowledge of local social service resources. * Ability to maintain confidentiality of records/information and establish priorities.
