The Senior Community Care (SCC) Pharmacy Team provides a variety of services including Institutional Special Needs Plan (ISNP/IESNP) strategic Medication Adherence initiatives and clinical support for other Part D measures such as Medication Therapy Management (MTM), and Statin Use in Persons with Diabetes (SUPD). The team also supports SCC providers via a clinical consultation, education, and deprescribing initiatives.

The Clinical Pharmacist will report to the Director of Clinical Pharmacist Patient Care within Optum Health. The primary responsibility of the Clinical Pharmacist will be to perform clinical chart reviews, End Stage Renal disease medication evaluations, identify drug therapy issues, influence appropriate prescribing, and improve member adherence. The pharmacist will be expected to make clinical recommendations for treating Heart Failure, COPD, and other common disorders. The pharmacist will participate in meetings with market clinical staff and provide therapeutic recommendations when necessary. The Clinical Pharmacist will be an integral part of a team of clinicians of varying disciplines working in collaboration to improve the well-being of our members.

Schedule: Monday - Friday, daylight shift

Primary Responsibilities:



  • Utilize Stars Part D reporting and internal electronic medical record to identify members who are non-adherent by looking at claims, past medical history, and disease states
  • Conduct clinical chart reviews and interpret clinical information (labs, problem lists, soap notes)
  • Present chart review findings to corresponding APC and work to optimize the members medication regimen
  • Provide education to both Clinical and non-clinical internal and external partners
  • Provide clinical education on Disease State Management
  • Lead meetings to review and improve medication adherence performance
  • Participating and engaging on a team of other clinicians both in person and virtually
  • Ability to work independently on projects and meet project deadlines
  • Participate in meetings to determine strategies to meet goals for Part D measures
  • Maintain Part D Measure knowledge and incorporate new measures into strategic plan as they arise
  • Work with PBM to have medications inappropriately billed reprocessed through the member's part D plan
  • Support projects with internal and external clinical collaborators
  • Maintain a high level of clinical knowledge of chronic disease states and changes in pharmacy practice in adult and geriatric patient populations to serve as an authoritative information source on drugs and their utilization in therapy

Required Qualifications:



  • Doctor of Pharmacy degree or equivalent experience
  • Active, unrestricted pharmacy license
  • 5+ years of Pharmacy Practice experience to include 2+ years Clinical Practice experience
  • Experience in a clinical setting, such as: Managed Care Organization, Clinic, Hospital, Consultant pharmacy, or Long-term Care pharmacy
  • Comfortable making clinical recommendations based on available clinical guidelines


Preferred Qualifications:



  • Completed Residency
  • Board Certification
  • Willingness to obtain pharmacy licenses if needed in restricted states
  • Proficient with Microsoft Word, Excel, Outlook, PowerPoint
  • Proven excellent written and verbal communication and relationship building skills
  • Proven ability to manage multiple complex, concurrent projects

The salary range for this role is $89,800 to $176,700 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
