The Med/Surg ICU Physician Assistant (PA) is a member of the multidisciplinary Med/Surg ICU provider team with patient care responsibilities. The Med/Surg ICU is comprised of medical, emergency surgery, general surgery critical care patients. The multidisciplinary care team consists of intensivist attendings from Surgery, Anesthesiology, Pulmonary Medicine, and Emergency Medicine and advanced practice providers.
Med/Surg ICU PA reports to the Med/Surg ICU Senior PA Manager, Medical Director and administratively to the Administrative Director of Pulmonary and Critical Care Medicine. Daily supervision is provided by the attending intensivist covering the Med/Surg ICU.
The Senior PA for Education and Training in the Med/Surg ICU will, in conjunction with the Senior PA Manager, work to promote the MSICU PA service within the Med/Surg ICU and to help guide its strategic direction. The Senior PA will take on a leadership role with the primary goal of increasing the collective and individual knowledge level of PAs within the Med/Surg ICU. Clinically, this individual will serve as a PA for the ICU and will assist the Attending Physicians in all areas of patient care. Key administrative responsibilities will center around Education and Training initiatives.

In addition to the responsibilities listed above, key administrative responsibilities will center around Education and Training initiatives including the following:

  • Maintain the ongoing educational lecture series and promote engagement and participation from the Med/Surg ICU PA team

  • Advance the educational curriculum to serve all PAs in the Med/Surg ICU to promote continued learning and maintaining up to date knowledge in the management of critical care

  • Serve as the lead PA for teaching and assessment of procedural skills, ensuring PAs achieve and maintain clinical competency required for each skill.

  • Provide necessary clinical feedback to the Med/Surg PA team in coordination with the Senior PA Manager and Medical Directors

  • Complete annual reviews of each Med/Surg PA's clinical performance

  • Develop a STRATUS PA Education framework for the Med/Surg ICU with basic and advanced skills to serve as part of onboarding as well as ongoing educational activities.

  • Maintain the Med/Surg ICU training modules including clinical and procedural competency benchmarks. This will serve as an onboarding document and allow for ongoing assessment of PA skills.

  • Implement best practices through representation on National Committee's such as the NCCPA and AAPA.

  • Support ongoing development of training programs and execute on individuals plans as defined by the Med/Surg ICU leadership.

  • Additional administrative responsibilities managing periodic staffing shortages by arranging appropriate coverage for shifts in the Med/Surg ICU one week each month, in coordination with the Senior PA Manager.

  • Additional administrative responsibilities may vary over time. Any proposed changes will be discussed with the Senior PA Manager and Medical Director if needed

In addition to these responsibilities, the Senior PA for Education and Training should display the following skills and take part in the following tasks:

Clinical Duties:

  • Performs patient admitting histories and physicals, records information accurately in patient record in a timely fashion.
  • Performs bedside procedures, e.g.: insertion of lines and tubes; performs postoperative wound care and removal of tubes, lines, sutures, etc.
  • Examines and evaluates patients and assists in the development of their treatment plan with attending intensivists.
  • Appropriately responds to emergency situations; manages patient issues as they arise in the ICU, to the extent that the PA will obtain and evaluate studies/labs, write orders, obtain consultations and interpret lab values and x-rays with the assistance of the supervising physicians as necessary.
  • Provides age specific care to medical/surgical intensive care patients from early adolescence through the geriatric ages.
  • Will work under the direction of supervising critical care attendings.
  • Provides teaching to medical students and physician assistant students and other health care personnel as appropriate.
  • Stays up to date on the latest literature in surgical critical care practice.
  • Participates in ongoing quality improvement, patient safety, and research efforts as directed by the service leadership. The PA should also initiate these efforts as appropriate.

Non-Clinical Duties:

Leadership

  • Mentoring of members of the MSICU PA team
  • Being supportive of team members and offering additional help when needed.

  • Attend continuing medical education courses as appropriate.
  • Practice professional behavior at all times.
  • Engage in activities to develop growth of self, team, and service.
  • Operations Support, Administrative Responsibilities
  • Keep proficient in all IS systems appropriate to position.
  • Maintain awareness of documentation and its effect on reimbursement.
  • Continue to find ways to practice efficiently.

WORKING CONDITIONS:

Work areas will include the Med/Surg ICU. This job requires exposure to potentially infectious material and the use of needles and other sharp instruments.

PA team members will alternate scheduled shifts for patient care in the BWH Med/Surg ICU, including nights, weekends, and holidays. The PA's responsibilities will be physically demanding and may require shifts exceeding 12 hours. Hours per shift and shifts per week may vary week-to-week.

SUPERVISORY RESPONSIBILITY:

The PA, while not directly overseeing others, should take on leadership responsibilities as follows:

  • Assist in orientation and training of new hires. Be available for questions and support.
  • Assist in orientation, training and mentoring of students, when applicable.
  • Support other team members' performance and development.

FISCAL RESPONSIBILITY:

The PA will be responsible for accurate, thorough, and compliant documentation to support proper reimbursement of encounters and procedures.

QUALIFICATIONS:

Education:

  • Must have successfully completed a Graduate Level, accredited Physician Assistant Program approved by AMA Board of Approval and Certification of PA programs.
  • Must have bachelor's degree or equivalent. (BS/MS joint programs are also acceptable).

License/Certification/Registration:

  • NCCPA certified.
  • License as a Physician Assistant in Massachusetts.
  • Maintain current DEA and Massachusetts Controlled Substance Licenses.
  • Maintain current BLS/ACLS certification.
  • Complete 100 hours of CME according to national and state regulations during each 2-year period of employment.
  • Complete BWH Professionalism Program within 3 months of date of hire.

Experience

  • Previous experience in an academic Medical/Surgical ICU environment strongly preferred.
  • Four years full-time experience as a PA.
  • Three years' full-time experience in critical care preferred.
  • Previous administrative, onboarding, training and/or supervisory experience preferred

  • The PA shall demonstrate the following clinical knowledge, skills and competencies:
  • Excellent patient-care skills.
  • Strong competency for the evaluation and treatment of critically ill patients.
  • Ability to learn, perform, and assist with a variety of technical procedures requiring excellent manual dexterity.
  • Strong clinical knowledge of surgery, medicine and critical care.
  • Knowledge and skills necessary to provide care to adolescents and adults, ages 13 and above.

Physical and mental abilities required:

  • Proven ability to be a productive member of a cohesive team of peers and multidisciplinary colleagues, dedicated to providing optimum patient care.
  • Ability to recruit, train, develop, and supervise PA team members.
  • Professional and appropriate attitude towards patients and families at all times.
  • Commitment to quality care, ethical judgment, and patient privacy.
  • Highest degree of professional judgment and discretion.
  • Excellent organizational skills and ability to prioritize duties appropriately.
  • Strong leadership and communication skills in order to work effectively with other members of the healthcare team, including physicians, trainees, dietitians, nurses, pharmacists, and administrative staff.
  • Understanding of procedures, policies and documentation required to ensure compliance with standards of care.
  • Receptive to feedback and strong ability to translate direction into improvement.
  • Understanding of own limitations in knowledge, skill, and experience.
  • Punctual and reliable.

The PA shall demonstrate the following professional knowledge and skills:

  • Proficient with BWH computer systems as needed to independently perform daily tasks such as order entry, EMAR, lab tests and results, incident reporting, etc.
  • Consistently provide high quality and timely documentation, including admission notes, progress notes, transfer notes, discharge summaries, and dictation.
