Job Description

* This is a new role ( but backfilling departed APC positions ) and is necessary to:
* Ensure high quality care
* Improve in-basket turn-around time and conversion of medical-decision making tasks into visits
* Trial adding APCs to JFMA in non-empaneled care roles as a model for centralized in-basket and access support
* Team-based (will be paired with team 1 and 3 in JFMA)
* Coordination with RN team and physicians in team model
* Each APC will be a team partner with 5-6 faculty clinicians on two clinical teams at JFMA
* Patient communication
* Team-centered care for acute access needs, scheduled follow-up, and communication needs
* Expansion of access, improved communication turn-around time, and improved clinician wellness
* Teamwork with JFMA RN's and MA's - focused on identifying on-the-fly visits and resolving patient concerns that require medical decision making tasks

* In-basket improvements
* Reduce unnecessary tasks (Rx refill protocols, message direction to non-clinical staff)
* Transparency between APC/MD team members
* Establish metrics of in-basket expectations for clinicians
