Overview

The Ambulatory Care Pharmacist provides direct patient care and comprehensive medication management for a wide variety of patients with chronic health conditions in the ambulatory setting. The pharmacist analyzes clinical data from multiple sources and utilizes best-practices to assist in developing pharmacotherapy recommendations that promote high quality, evidence-based, cost-effective pharmaceutical care. The Ambulatory Care Pharmacist provides direct patient care related to medication prescribing, administration, and monitoring within a care team model and provides clinical expertise to assist the care team in completion of their duties. The Ambulatory Care Pharmacist is responsible for support of providers with the medication reconciliation process and will provide education to providers and patients in the outpatient ambulatory setting. Pharmacist will provide ambulatory pharmacy services to locations as assigned and may need to provide coverage to or float to multiple clinics.
Qualifications

Certifications: Licensed Pharmacist - Illinois Department of Financial and Professional Regulation (IDFPR), Education: Bachelor's Degree: Pharmacy; Experience In Lieu of Education: Pharmacy; Doctorate: Pharmacy, Work Experience: Clinical pharmacy
Responsibilities

Provide comprehensive medication managment including conducting thorough medication reconciliation, performing clinical interventions, working to reduce polypharmacy, reduce and identify adverse drug reactions, and refer patients to pertinent clinics (i.e. anticoagulation).
Provide patient monitoring for the purpose of evaluating and optimizing patient outcomes, drug utilization, and patient compliance, detecting and minimizing ADRs, and eliminating unnecessary drug use and duplication.
Involved in development, implementation and evaluation of patient care services in the ambulatory setting.
Performs medication utilization reviews.
Serves as drug information resource for other healthcare providers and departments.
Maintains current knowledge of and ensures compliance with government and commercial regulations.
Conduct comprehensive medication reviews at the request of provider.
Serve as a subject matter expert for various committee or subcommittees requiring pharmacy knowledge.
Work in collaboration with other healthcare professionals.
Provide Medication Therapy Management services as defined in the Illinois Pharmacy Practice Act
Assist in the training and orientation of new employees as assigned.
Other duties as assigned.

Compensation and Benefits

The compensation range for this position is $51.41per hour - $88.43per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
