Summary:
The Case Management Assistant supports case managers by performing administrative duties, coordinating care plans, and facilitating communication between patients, families, and healthcare providers.
Responsibilities
Coordinates follow-up appointments for patients.
Assists with the discharge planning process, including arranging home care services, skilled nursing facilities, durable medical equipment, etc.
Provides administrative support such as scheduling meetings and managing correspondence.
Participates in ongoing training and development activities.
Delivers regulatory forms to patients/authorized representatives.
Scans and enters documentation into EMR and updates document-controlled forms.
Orders supplies and manages finances for the department.
Answers phone calls and redirects to the appropriate extension.
Manages department mail, faxes, and emails.
Prepares minutes from meetings.
Assists with Joint Center Orientation.
Other information:
Required
High school diploma or equivalent.
One (1) year of administrative experience in a healthcare setting.
Proficiency in using electronic medical recordkeeping (EMR) systems.
Basic Life Support (BLS) for healthcare provider certification.
Strong organizational and communication skills.
