Hourly Pay Range:

$36.00 - $55.80 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

RN Care Coordinator, Immediate Care Center

Position Highlights:

  • RN Care Coordinator - Immediate Care Center
  • Location: 15 S McHenry Rd. Buffalo Grove, IL.
  • Part Time: Per Diem
  • Days and Evening, Weekend as needed

What you will do:

  • Achieves quality patient outcomes by managing day-to-day care coordination and clinical management activities. Models and facilitates collaboration among the interdisciplinary team, i.e., physician, therapies, clinical nurse specialists, utilization coordinators, and other departments. Collaborates with interdisciplinary team to identify and resolve issues that impact team performance and the delivery of patient care services. Communicates and collaborates with others to provide continuous, smooth transition of care and day-to-day operations. Coordinates patient plan of care, based on area of service and role expectations, to assure interventions are consistent, safe, timely and appropriate. Assesses continuity of care and evaluates coordination of interventions by the interdisciplinary team to ensure consistency and compliance within the plan of care recommendations.
  • Makes assignments and manages staff throughout shift promoting continuity of caregivers. Monitors daily service activities, patient flow, and facilitates completion of assignments and deployment of staff. Provides oversight to ensure adequate unit supplies, equipment, and stock. Utilizes supplies and equipment in the provision of patient care in a manner, which assures quality and cost effective patient care. Informs assigned Supervisor of any service disruption involving staff, customer service, and/or process improvement opportunities to better meet the needs of the patient experience. Maintains a safe work environment in compliance with all regulatory agencies.
  • Provides patient care as indicated and appropriate to the scope of service area. Responsibilities may include, but not limited to the following:), IM antibiotics, immunizations, administration of nebulizer treatments, etc. Provide Nursing Visits and POC testing and oversees clinical staff performing these functions. (i.e. Pro times). Oversees site-specific CLIA and OSHA standards and has oversight for daily QI processes to ensure customer service and quality goals.
  • May also perform various OHS tests according to client specifications. Reviews OHS client database to ensure that the client company receives correct services for its employees. Performs substance abuse specimen collection, i.e., Breathalyzer testing (EBT), and Urine Drug Screens (UDS). Assists with other ancillary testing or mandated exams according to identified services and related guidelines. Processes Chain of Custody urine drug screen specimens for shipment to certified labs for testing, according to the client company's instruction.
  • Participates in evaluating quality and effectiveness of nursing practice by participating in quality improvement. Incorporates evidence based standards into nursing practice and modifies nursing practice as indicated. Maintains professional knowledge in nursing practice by participating in both mandatory and professional educational initiatives. Maintains clinical competencies as defined by areas of practice.
  • Interviews, hires, disciplines and discharges (when necessary) subordinate team members. Regularly evaluates the performance of team members in accordance with the hospitals performance management system. Supports development of team member's knowledge and skills through regular feedback, recognition of positive accomplishments and coaching opportunities.
  • Interacts with and contributes to professional development of peers and other health care providers as colleagues. Shares knowledge and provides feedback to contribute to an environment supportive of clinical education. Assigns or delegates tasks as defined by State Practice Act according to knowledge and skills of designated caregiver. Collaborates with physicians and interdisciplinary team using open dialogue for the provision of patient care.
  • Communicates and coordinates plan of care to patient and family regularly respecting the patient's wishes to share. Communicates patient data to physician and interdisciplinary team as indicated to change/initiate interventions as needed. Documents collection of patient health data, interventions and communications and is responsible for Case Management.
  • Initiates patient and family teaching as soon as patient demonstrates readiness to learn. Assesses patient's learning style and learning barriers and modifies teaching plan accordingly. Provides teaching on disease, management strategies, emergency care and drug information when appropriate. Identifies community resources to support patient self-management of disease.
  • Facilitates decisions and actions on behalf of patients in an ethical manner by maintaining patient confidentiality, acting as a patient advocate and assisting patients so they can advocate for themselves. Seeks available resources to support ethical practice, assists patient and family in becoming informed consumers about cost, risk and benefits of treatment and care.
  • Responsible for oversight of patient electronic medical record (EMR), Epic and Systoc documentation. Reviews clinical documentation for completeness and accuracy.
  • Demonstrates the knowledge and skills necessary to assess, treat, and provide care appropriate to the age(s) of the patients served. Maintains clinical competencies as established by areas of practice.
  • Adheres to all Northwest Community Healthcare standards, policies, and procedures.

What you will need:

  • Successful completion of a BSN from an accredited school of nursing.
  • Current licensure to practice nursing in the State of Illinois
  • Current CPR certification from the American Heart Association
