Location Information Saint Therese offers a continuum of care and services available in a person's own home or in one of our four Twin Cities communities: New Hope, Brooklyn Park, Shoreview, and Woodbury. Responsibilities Evaluates social factors relating to rehabilitation requirements and provides personal counseling and referral services to residents of the facility. Evaluates and attends to the psycho-social needs of residents. Qualifications Education: Bachelor's degree in Social Work, Sociology, or related field Experience: One to three years on the job experience (preferably in a long-term care setting). Experience with discharge planning preferred. Licensed Social Worker (in Minnesota) preferred. Special Knowledge, Skills, and Abilities:
