Community Health Coordinator - Medical Office North Bethesda, MD Pay From: $23 Per hour MUST: 1+year of experience working in the community health -Population Health field Experience reaching out to high risk patients by phone and email and in person Assisting with disease management and obtaining social services and therapies Experience collecting and tracking population health related information Handling community outreach Performing non clinical health assessments Supporting patients with self management of diseases and providing access to patient care Providing health education Assisting with providing community resources Bachelor's Degree in Public Health or Health related DUTIES: The Duties of the Community Health Outreach Coordinator include but are not limited to the following: Identify patients at high-risk for developing or living with diseases and perform health assessments Assist community members in accessing healthcare and Social Services programs and services Participate in community events and provide community outreach Create reports and collect and track population health-related information Target outreach to community members and build relationships that result in increased access to health care Support patients with utilizing disease self-management plans and assist with clinical and therapy support to improve health Assist the healthcare team with providing access to patient care and improving patient care experience Assist patients with clinical support, therapy and health education Reach out to clients via phone and email Quadrant is an affirmative action/equal opportunity employer.
