Nurse Practitioner or Physician Assistant - Pulmonary/Critical Care- Lung Transplant Program

Position Description:
The Department of Medicine, Division of Pulmonary and Critical Care is seeking a Nurse Practitioner or Physician Assistant to medically manage complex patients within our Lung Transplant Program located in Milwaukee, WI at Froedtert Hospital.

The Advanced Practice Provider (APP) works as a member of a multidisciplinary health care team who will contribute to excellence in program administration, patient care, patient safety, research, quality improvement, and teaching.

The Ideal Candidate will be a Pulmonary or Critical Care APP with a passion for Pulmonary/Critical Care medical management and an interest in being part of a thriving & very collaborative academic practice at the Medical College of Wisconsin (MCW). Areas of responsibility will include, but not be limited to providing direct patient care to patients in a blend of Inpatient & Outpatient settings.


In this role you will be responsible for the following
:

  • Provide care focusing on the Lung Transplant patients in a blend of inpatient & outpatient settings
  • Collaborate with interdisciplinary healthcare teams in diagnosis & treatment of conditions requiring medical intervention for inpatients
  • Facilitate and communicate with patients, families, care team, & staff to promote continuity of care across the continuum of care
  • Support inpatient & outpatient medical (non-surgical) rounding w/ Tranplant Director/Faculty
  • Transplant/Discharge teaching for patients during inpatient stay
  • Facilitate transition to outpatient care following hospital discharge
  • Provide exceptional, high acuity patient care
  • Attend sub-specialty specific conferences as appropriate
  • Assist in recruitment & consent of clinical research subjects as needed


Core Advanced Practice Responsibilities:

  • Perform comprehensive histories & physical exams, identify normal and abnormal findings including physical, cognitive, & psychosocial status
  • Order, perform, and interpret screening & diagnostic procedures and tests, and appropriately communicate & document information
  • Complete accurate & timely progress notes, procedure notes, and/or admission & transfer orders
  • Develop pharmaceutical & non-pharmaceutical treatment plans
  • Monitor effectiveness of plan of care
  • Educate patients on disease & treatment plans
  • Perform procedures as trained and appropriately credentialed
  • Document within the Electronic Medical Record (EMR), and completes other necessary documents
  • Collaborate and consult with physicians & other health care providers
  • Educate, counsel, and assist patients & families regarding current health problems, health maintenance activities, & preventive behaviors
  • Attend required training and meets key regulatory requirements, including those of the privileging institution, CMS, and the Joint Commission
  • Acquire information on the most current development & trends in the Physician Assistant field and/or Nurse Practitioner field, through educational programs, in-services, professional associates, and current literature

Schedule/Hours:

  • Primarily Monday-Friday, 8 am - 5 pm, and additional hours as needed

Preferred Experience:
Pulmonary, Lung Transplant, or Critical Care APP or RN experience highly preferred

Minimum Education:

Advanced Practice Nurse: Master's Degree in Nursing
Physician Assistant: Master's Degree in Physician Assistant Studies
