Care coordination provided for addiction services patients.

The coordinator uses systems of care processes in decision-making, patient assessment, development and implementation of care coordination methods and systems to improve the overall quality and efficiency of patient care for the designated department and population served. Provides direct and indirect patient care as needed; oversees the delivery of patient care; and provides day to day management of programs to coordinate patient care as designated by the department. Coordinates the delivery of safe, high quality patient care; identifies areas for study; and participates in and/or supports research. Works in compliance with all State, Federal, and other regulatory requirements. Serves as a resource and mentor to staff and team members and demonstrates excellence in interprofessionalism.

  • Organizes, formulates and coordinates a multi-disciplinary approach to the patient population forachievement of optimum outcomes.
  • Performs daily reviews of outstanding labs and radiology reports, and reports discrepancies to the Medical Director as outlined in follow up protocols.Performs telephone contact with patients to instruct on follow up and / or discuss understanding of individual service plans.
  • Attempts at a minimum biweekly contact with patients as described in the office-based opioid treatment protocol , or at the request of a Licensed Practitioner or Clinical Staff member for a specific concern.
  • Problem solves to facilitate effective outcomes and resolves complaints, issues, or concerns.Provides service recovery as needed.
  • Maintains a thorough knowledge of the office-based opioid treatment clinical information systems for documentation in the EMR, telephone encounters and generating monthly reports.
  • Develops, collects, analyzes and evaluates data as needed to identify trends. Presents findings and makes recommendations for performance and quality improvement, JCAHO/regulatory agency requirements, and tracking patient outcomes aimed at accomplishing department and organization goals.
  • Serves as advocate for patient, protector of patient's rights, and provider of psychological/social/cultural support.
  • Provides a high level of customer service to patients, clinical staff and colleagues.
  • Adheres to patient confidentiality policies and other regulatory requirements.
  • Demonstrates the ability to work independently with minimal direction, prioritize and meet deadlines.
  • Provides age appropriate education on an as-needed basis to patients during contact conversation..Education is specific to patient need, and will be well documented.
  • Education is incorporated into the plan of care and includes physicians, staff, population specific representatives about patient care options.
  • Serves as a liaison between department staff, patients and families,physicians, external vendors/partners to ensure a collaborative approach to care for patients and families.
  • Interacts with staff, patients, physicians, families, visitors, and general public, representing Carilion Clinc during all interactions.
  • Communicates any concerns or complaints, or if unable to achieve satisfactory service recovery to managers.
  • Participates in department and/or system performance improvement initiatives as requested.
  • Uses documents, spreadsheets and other tools as needed.

Hours: Monday-Friday 8:00am-5:00pm

What We Require:

Education: Bachelor's degree required in a human services, counseling, social work, psychiatric rehabilitation, psychology, sociology or vocational rehabilitation as needed to satisfy the current DMAS and licensing standards per the Board of Health Professions which at this time requires one year of clinical experience with substance abusing population.

Experience: Minimum of 1 year substance abuse clinical experience in providing direct services to persons with a diagnosis of mental illness or substance abuse; or one year of clinical RN experieince required.

Licensure, certification, and/or registration: Current Virginia RN license required, if an RN, or A Certified Substance Abuse Counselor (CSAC) or a CSAC-Assistant under supervision prefered. De-escalation training must be successfully completed within one year of hire and then maintained going forward. BLS-HCP required within 1 year of hire or at time of hire.

Other Minimum Qualifications: Effective interpersonal, communication, and team-working skills required. Strong critical thinking, conflict resolution, organization and effective judgement abilities required. Must successfully complete orientation and competency validation requirements. Must be able to meet essential job functions.Designated areas may require Knowledge of Medicare/Medicaid and other third party payor regulations.
