Position Summary: This role reports to the Palliative Director and will help maintain the Palliative census at levels that the department is staffed for, provide initial outreach to members to explain Palliative services and schedule Initial Palliative visits for the assigned clinician. The RN in this role will also be reviewing the Cyft Smart List to review members identified with Palliative need in the CCA electronic health records to clinically validate findings and assure that the member is in the home or SNF setting where Palliative team members can visit in-person to perform assessments and coordinate the referral to the Palliative program. This role will complement the data-driven outcomes the Palliative department achieves by tracking outcomes, opportunities and developing KPIs to achieve best results through feedback to the vendor and utilizing and developing unique reports for tracking outcomes and progress towards them. Supervision Exercised: This position has no direct reports Essential Duties & Responsibilities: * This role requires superb communication skills, the ability to speak with members and their caregivers in a health-literate way as well as with internal and external providers. * Requires extensive knowledge about Palliative vs Hospice care criteria, the ability to educate and support partnerships with external and internal customers. * Review CyftList members identified with Palliative need in the CCA electronic health records to independently clinically validate findings for Serious Illness presence and Palliative care needs. This assimilation will need to be communicated to PCP and CP in a succinct way and tracking for approval needed to proceed with the admission. * Will identify any opportunities within the CyftLIst for improvement to identify members with Palliative need more accurately and work with the vendor to implement changes. * Assure that referred member is in the home or SNF setting where Palliative team members can visit in-person to perform assessments. Tracks outcomes and opportunities within different facilities to promote more effective member care. * Perform initial outreach to member or healthcare surrogate to explain Palliative Care program at CCA, obtain verbal consent for Initial visit, schedule the same with the member or healthcare surrogate and assign clinician in coordination with clinician's supervisor. * Participate as part of the Palliative Leadership team to independently track and assimilate conclusions around gaps in population health needs for the members served. Identifies process improvement opportunities and implements solutions * Will also develop method for tracking data on admissions, outreach attempts and findings related to demographics and report them to the Palliative Director. * Will assimilate trends on the populations in areas served, both of the members who successfully enroll in Palliative care as well as those who don't. * Will identify quality improvement projects specific to population health for admissions and initiate KPIs to improve performance. * Provide clinical care to members via telehealth technologies (video, chat, etc.) for a clinically appropriate clinical care and care management services. * Other projects and requests as made by the Palliative Director Working Conditions: Standard office conditions. Non-Member facing Education Required: * Minimum of bachelor's degree in nursing Education Desired: * Hospice and Palliative care certification Experience Required: * 5+ years' experience in Hospice or Palliative care Experience Desired: * Previous experience doing admissions, chart review * Microsoft Office knowledge; at least a basic proficiency in Excel, PowerPoint, and Outlook * Expertise speaking to physicians and other disciplines, must have professional and advanced communication skills to establish partnerships * High level of organizational skills * Ability to prioritize * Ability to operate in a self-directed manner by exercising clinical judgement to enhance member care. * Willing to learn and utilize telehealth technologies (video, chat, etc.), when appropriate, for a variety of clinical care and care management services. Language Required: * English Language Desired: * Bilingual in Spanish strongly preferred
