Job Summary

Coordinates program planning, implementation and evaluation in area of accountability in collaboration with members of the health care team. Provides educational support for patients/significant others, health team members and the community, and coordinates process improvement studies to measure outcomes of care.

Essential Functions

In addition to core responsibilities, the Community Health Program Specialist will support the West Market through the following key functions:

Program Coordination & Technical Support

  • Serve as Community Benefit Coordinator, sharing best practices with reporters.
  • Co-develop standard work documents and co-facilitate office hours with the HEEAAT Specialist.
  • Provide technical assistance to regional reporters.

Compliance & Reporting

  • Lead compliance tasks, including 990 Schedule H narrative development and CBISA entry reviews.
  • Collaborate with service line managers to enhance reporting and recruit new reporters.

Data & Impact Reporting

  • Synthesize community benefit data for governance, leadership, and the annual Community Health Impact Report.

Administrative Oversight

  • Manage CBISA access, entry approvals, and project timelines.
  • Maintain cross-functional coordination with regional teams.

Grant & Award Management

  • Coordinate the Community Building Award and Grant Program, including application review, award tracking, reporting, site visits, and documentation.
  • Support the development and maintenance of grant-making systems, processes, and protocols to ensure transparency and efficiency.
  • Collaborate with department leaders to identify funding priorities, align awards with strategic goals, and evaluate impact.
  • Build and sustain partnerships with internal stakeholders and external community organizations to strengthen engagement and outreach.
Qualifications

Required

  • Bachelor's Degree Social Work, Education, Psychology, Community Health or Nursing; or equivalent education and experience
  • 3 years of relevant experience program planning
  • Valid Driver's License
