Per Diem/As Needed

We are currently seeking a Discharge Planning/Case Management/Utilization ReviewRN to join our team. This is a per diem, day shift (as needed) position working8-hourshifts.Weekend requirements may be every third weekend. In addition to the requirements listed below, our ideal candidate has a strong medical/surgical, intensive care or home health care background.

Previouscase management andutilizationreview experience is needed.

Summary:

The Registered Nurse - Case Manager - MMC BID SAN role provides patient-focused coordination, negotiation, and procurement of necessary resources to address all identified areas of need for facilitation of achievement of quality, cost-effective outcomes. Acts as a patient advocate through collaboration with the patient and the interdisciplinary staff, internal and external to the organization.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  • Education: Graduate of an accredited School of Nursing required; Bachelor's Degree preferred.
  • License/Certifications: Current applicable state(s) license as a Registered Professional Nurse required.
  • Experience: Five years of acute care clinical setting nursing experience preferred. Knowledge and experience in discharge planning, case management and utilization review preferred.
