Care Coordinator

Access Services

General Summary: Care Coordinator provides care coordination in consultation with the medical and social service providers, to complete screenings, case management and resource coordination, seeking to understand and/or alleviate mental health and/or substance use distress and barriers to accessing needed supports. The coordinator carries a caseload that enables consistently meeting CCBHC program requirements, collaborates with other Thrive providers and community providers as well as outside agencies; adheres to agency, state, federal, and licensing board guidelines regarding ethical practices, billing, and documentation.

Essential Responsibilities:



  • Coordinates internal and external referrals.
  • Utilizes aspects of the SBIRT model (Screening, Brief Intervention, and Referral to Treatment), that includes referrals to treatment between Thrive and other community partners.
  • Assists individuals and families with identifying and prioritizing goals
  • With individuals, formulates time-limited care coordination plans that target clearly identified care coordination goals with a means of measuring success and program completion.
  • Provides care coordination and resource connection
  • Establishes relationships with community team, other providers, partnering community agencies, and identified support systems for clients, collaborating as indicated for clinical reasons.
  • Advocate and provide linkages with community resources to address barriers to treatment.
  • Completes periodic reports for the agency and state as assigned.
  • Complete billing forms, logs, data collection forms, progress notes and other clinical documentation in accordance with Thrivestandards.


Additional Responsibilities:



  • Attend all agency/department required training and meetings within the prescribed period.
  • Attend all Safety Training programs and be able to describe his/her responsibilities related to the general safety, department safety, and specific job-related hazards and responsibilities.
  • Demonstrate respect and regard for the dignity of all clients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment.

Qualifications

Education, Experience and Competencies:



  • Bachelor's degree in Psychology, Social Work, or Human Services related field.
  • Minimum of one-year work experience in the behavioral health field or related social service field, preferred.
  • Excellent service coordination skills and able to build team support.



Certifications, Licenses Requirements:



  • Valid driver's license and automobile insurance which meets Rhode Island minimum standards required, or the ability to obtain necessary transportation in order to perform the responsibilities/tasks of the job.
