Care Coordinator Access Services General Summary: Care Coordinator provides care coordination in consultation with the medical and social service providers, to complete screenings, case management and resource coordination, seeking to understand and/or alleviate mental health and/or substance use distress and barriers to accessing needed supports. The coordinator carries a caseload that enables consistently meeting CCBHC program requirements, collaborates with other Thrive providers and community providers as well as outside agencies; adheres to agency, state, federal, and licensing board guidelines regarding ethical practices, billing, and documentation.
