- Hourly compensation between $19.00 - $21.00, PLUS an annual performance-based, discretionary incentive. What You Will Do - Essential Functions
The Care Coordinator (CC) is a critical member of the clinical care team. He/she is office-based and responsible for providing administrative support to the clinical team as they provide cost-effective, high-quality health care for patients. Responsible to the clinical team and reports to the Care Coordination Supervisor.
- Administrative and Communication Support
- Provide administrative support to the clinical care team
- Answer incoming phone calls for general inquiries and redirect as appropriate
- Route incoming faxes and assign as appropriate
- Support scheduling appointments and reminder calls or texts
- Care Coordination and Patient Support
- Assist with the implementation or finalization of patient orders that do not require an RN to complete
- Assist with referrals and referral management
- Assist with follow-up to assure needed DME is ordered and delivered
- Provide oversight for all Care Coordination functions including but not limited to orders, referrals, disenrollment, scheduling, error processing, medical records and consent uploading, mail retrieval and sorting, email processing, etc.
- Medical Records and Data Management
- Facilitate medical records management for outside documents that are critical for patient management (e.g., lab results, discharge summaries, documentation from specialists and PCPs)
- Support a variety of data entry functions
Who You Are
- Required Qualifications
- Minimum of a High School diploma
- Exceptional verbal and written Communication skills
- Understanding of medical terminology
- Completion of a Medical Terminology course preferred
- Medical office experience preferred
- Strong work history
- Working knowledge of computers and ability to manage data within an electronic system
- Five years of work experience preferred
- Able to be on the phones throughout the day

- Participation in daily huddles and virtual team/company meetings
