The Opportunity:

One Medical is seeking a Social Programs Manager who is a Social Worker with extensive experience in complex care navigation or community case management within High Risk Care (HRC) Programs. Reporting to Manager, High Risk Field Operations, this individual has the ability to provide sound social work and social services leadership within their market and nationally. They will serve as a key member of the team to extend and enhance the expertise and impact of our social workers and social programs teams. This leader will spend approximately 30% of their time on leadership activities and 70% on direct clinical care.

What you'll work on:


  • Provide direct clinical care and resource management for patient panels based on staffing support needs to support overall care goals within the High Risk programs
  • Develop, implement, and evaluate evidence-based practices for chronic illness, palliative care, and case management
  • Build and maintain program infrastructure, which includes standardizing workflows and documentation process
  • Supervise and mentor social worker and social program team members
  • Manage clinical care standards by ensuring quality metrics and effectiveness through regular audits
  • Serve as a key stakeholder in the recruiting, hiring, and onboarding process for new team members

What you'll need:


  • At least 5 years of clinical experience, 3 or more as a leader in a High Risk Care program or similar programs such as palliative care programs or complex care navigation programs
  • LCSW or MSW with active license in either CO, AZ, GA, WA or TX AND willingness to obtain other licenses or compact license
  • At least 2 years of managing teams


Physical Space Requirements:


  • HIPAA compliant area within home that is secure, quiet and isolated from others to protect PHI
  • Reliable internet connection


This is a full-time, remote position in which the person must be willing work Monday - Friday, 8:00am - 5:00pm in Pacific or Mountain Time Zone, regardless of virtual location.
